If you do not have one, please follow the website instructions below for Customer
Registration.


                       “Application for Service” Instructions

   1.   GO TO YOUR WEB BROWSER AND TYPE IN THE ADDRESS WWW.CPLINK2.COM

   2.   GO TO “BECOME A CUSTOMER”(If you do not have Adobe Acrobat Reader, please
        follow the instructions on the screen before proceeding to Step 3)

   3.   GO TO “START”

   4.   FILL OUT ALL NECESSARY FIELDS REGARDING YOUR AGENCY.

       Please note that the Contact Person must be someone legally authorized to fill out the
        application for your agency.
       If you wish to establish additional users, this application process will print out a blank user
        page to be used to create the additional users.
       Once you arrive at the section “Sponsor Information”, if you do not have a Sponsoring
        Insurance Company, you can fill out “agency bill” and put “none” for agency code (all
        reports ordered will be billed to your agency).
       If you have a Sponsoring Insurance Company, the agency code is the agency/producer code
        that is assigned to your agency by the sponsoring insurance company. If you don’t have one
        assigned by your sponsoring company, you may put “none”.
       Each agency location is required to complete the agreement and all states forms.

   5.   “DOWNLOAD APPLICATION WITH STATE FORMS” (If any required fields have
        missing information, you will receive an error message “validation error”. Please corrects
        the error(s) and “DOWNLOAD APPLICATION” again).

   6.   ONCE THE APPLICATION IS DOWNLOADED, all forms will be pre-filled with your
        agency information. You will only need to sign the forms and fax them to ChoicePoint.

   7.   Print application, read and sign all forms. We are required legally to have all forms signed
        and completed before proceeding with your account setup.

   8.   The application can be faxed to 770-752-3601. Please include a copy of your insurance
        license.
