Application and Procedures
Nomination Procedures
The Illinois Scholarship Program (ISP) Scholarship is funded by a grant from the Grand Victoria
Foundation, Chicago, Illinois. The “program” consists of ten, $2,000 scholarships and career
and leadership development activities for recipients. Each recipient will receive a $2,000
scholarship, payable in two installments of $1,000 each: Fall 2005 and Winter/Spring 2006.
Scholarship recipients will be designated Illinois Scholarship Program (ISP) Scholars.
Nominations are made by directors of TRIO/EOP programs located at Elgin Community
College, College of Lake County, Family Centered Education Agency, Chicago State
University, Northern Illinois University, South Suburban College and Rock Valley
Community College.
The TRIO/EOP directors verify all applicant information. The director must complete a
nomination form which includes a recommendation and forward to the MAEOPP Scholarship
Committee, which, in turn, will forward to the ILAEOPP Scholarship Committee (see below).
The complete committee is comprised of the Illinois Association of Educational Opportunity
Program Personnel (ILAEOPP) Scholarship Committee (part of the MAEOPP Scholarship
Committee) and the MEF Scholarship Committee.
The postmark deadline for the application and nomination forms and supporting
documentation is June 30, 2005. Mail to:

Eligibility Criteria
Applicants for the Illinois Scholarship Fund should be:
A) A TRIO/EOP program participant who is low-income and/or a first-generation college student.
B) Enrolled as a high school senior with at least a 3.0 GPA on a 4.00 scale, or currently enrolled in a 2-
year or 4-year college with at least a 2.8 GPA on a 4.00 scale, and is in their freshman through junior
year.
C) A student who can commit to completing a community service project which is described within this
document.
D) A student who can commit to attending a leadership and career development activity which is
described within this document. Expenses will be paid by the MAEOPP Education Foundation.
E) Willing to provide all information to meet the Winter/Spring 2006 semester criteria with regard to
proof of enrollment and satisfactory academic progress.
F) A participant in a Trio or EOP program at the following institutions: Chicago State University, College
of Lake County, Elgin Community College, Family Centered Education Agency, South Suburban
College, Northern Illinois University, or Rock Valley Community College.
G) A student whose goal is to obtain a baccalaureate degree from a four-year college or university.
H) Enrolled and completes status as a full time student (minimum 12 hours credit each semester) for the
Fall 2005 and Spring 2006 semesters.
The complete application package will include:
1) A signed and dated Illinois Scholarship Program (ISP) Scholarship Application Form
2) A typed Student Narrative/Essay addressing
a statement of personal character
evidence of community and/or leadership involvement
evidence of multicultural activities and contributions
student’s commitment to academic excellence
student’s college and career goal(s)
student’s plan to give back to the MAEOPP organization, sponsoring TRIO program, high school, college
or university, or local community
3) A nomination form from the Program Director
4) The most recent high school or college transcript
Information needed upon notification of award before the first check is sent:
Recipient must submit a current photograph (head shot preferred) for publication. A digital photo is also
acceptable. (Send digital photo(s) attached to an email to: rwarner454@aol.com)
Prior to receiving the actual check students must submit verification of full time enrollment (a minimum of 12
hours for grade) for Fall 2005.
It is the student’s responsibility to ensure that all forms are signed, dated and the transcript is included in the
application packet and given to the Program Director. The Program Director will submit the completed
application to the Chapter Scholarship Chair by the deadline postage date of June 30, 2005.

Service Project Guidelines
For Completion By the Winter/Spring Semester 2006
Who Must Complete a Service Project
All recipients of the Illinois Scholarship Program (ISP) Scholarship must complete a service
project between the fist and second semester in college. Payment of installment two of your
scholarship is dependent upon the completion of a pre-approved service project by the
chairperson of the MEF Scholarship Committee.
Purpose
The purpose of the service project is to reinforce the practice of helping others who are in need
and enhance the leadership skills of recipients. This is central to TRIO, MEF and the spirit of the
ISP Scholarship.
Project Duration
There is no set project length. However, the project must be completed before the second
installment of the scholarship is paid. Also, there should be meaningful benefits from the
services provided and duration of service is often central to the benefit stream. The service
project must be completed and the final report submitted by no later than January 15,
2006.
Impact
Service projects must have a measurable or discernible impact. Each Service Project Supervisor
shall be responsible for evaluating the impact of services provided by the ISP Scholar.
Types of Projects
Acceptable service projects include, but are limited to, the following:
1. Volunteering as a tutor or mentor for your local elementary or high school
2. Serving as mentor for students in other TRIO programs, i.e., Upward Bound, Talent
Search, etc.
3. Working with a local Boys or Girls Club
4. Assisting local churches with their service projects, i.e., a breakfast program for
young children
5. Volunteering at a local shelter for battered women
6. Initiating/participating in a voter education project for teenagers/college students
7. Organizing a local chapter of the TRIO Association
8. Developing and presenting a workshop at your state TRIO Conference, the MAEOPP
Student Leadership Conference
9. Rallying students to support TRIO and Student Financial Aid at the state or national
level
