In order for the Certificate of Visa Eligibility form (I-20 or IAP-66) to be issued, it is necessary to submit complete and accurate information regarding your sources of financial support. This information is required by the University of Illinois at Urbana-Champaign in compliance with regulations set forth by the U.S. Immigration and Naturalization Service (USCIS) for all students planning to enter the United States under its auspices. You are required to submit current evidence that you will have at least as much money as is described below for the entire length of your program. Financial support must be liquid assets.
All financial documents used for verification must include these elements:
• • • • • • • • An ORIGINAL document on official letterhead -- no copies or faxes can be accepted An original signature of a bank official An official bank stamp and/or seal The type of account must be clearly stated, for example “checking”, “demand savings” The exact balance of the account in numeric terms The document must be less than one year old If not in English, the document must be accompanied by an official translation (Internet bank statements will only be accepted when stamped and signed by a bank official)
The evidence of financial resources that we are requesting you to present is frequently required by U.S. consulate officials before a visa is granted. The current estimated cost of study at the University of Illinois at Urbana-Champaign is described below. These are estimated expenses for a single student not accompanied by dependents. Program tuition and fees are set annually by the University Board of Trustees and are subject to change without notice. Rate changes are usually effective at the start of the summer term of each academic year.

For Married Students
If you are married and will be accompanied by your spouse and/or children, you must certify to the U.S. embassy or consular office that you have additional financial resources before dependent visas can be issued by that office. The additional expense for your spouse (one dependent) is estimated to be $517 per month ($6,200 per year). The expense for two dependents is approximately $832 per month ($9,980 per year); and each additional dependent is $160 per month (1,920 per year). You are also encouraged to purchase health insurance for accompanying dependents; the current yearly rate for University of Illinois health insurance is $3,096 for the spouse and $1,542 for children. These rates are subject to change.
All documents must be original (no photocopies or fax copies accepted). You will not receive an I-20 or IAP-66 until this information has been received by the University.

Personal: If you, a member of your family, or another individual will provide the funds for your educational and living expenses, you are your own sponsor and must complete the following financial statement. Attach verification -- a bank statement or other evidence of cash available and transferable. Verification documents must include the elements outlined on page 1 of this form. You are encouraged to use the SAMPLE FINANCIAL AFFIDAVIT located on the MS-Tech website to produce this document. Other/Sponsorships (government, business agency, grant): If you will be sponsored by a government, business agency, private grant, or scholarship, your sponsor must complete the following financial statement and attach the official dated, signed letter of sponsorship (on letterhead) when returning this form stating that the sponsor agrees to provide funding for the full cost of tuition, fees, and living expenses, including dependents.
