                   Request for Transfer Credits – Instructions


The attached form can be used to request approval for transfer credit. You
do not need to have an advisor sign the form.

Please return the form to janice.keivel@notes.udayton.edu, or feel free to fax
it to 937-229-3392.

Remember, a maximum of six (6) semester (or 9 quarter) hours of graduate
level coursework, completed at another university, can be transferred into
the master’s degree program if the coursework has been completed within
the last five (5) years with a grade of “B” or better.

You must also submit (via email or fax) a copy of the transcript and a course
description or syllabus of the course you are requesting be transferred. Prior
to approval, an official transcript will be required if one was not submitted at
the time of admission.

If you are pursuing a licensure and not a degree, you DO NOT request a
transfer of credits. Credits accepted towards your licensure program will
be noted on your evaluation checksheet which is prepared at the time of
admission.
