OVERVIEW:
Container Technologies Industries, LLC (CTI) is committed to providing the best possible service to our
customers. In an effort to achieve that end, it is necessary for us to have a complete and clear
understanding of customer requirements. CTI has compiled a detailed product specification for the
purchase of containers. This form will assist our customers by listing the information necessary to meet
requirements needed by CTI to process your Request for Quote (RFQ) or Purchase Order (PO). To help
you answer unfamilar questions, we have listed CTI's standard for each specific criteria. It is very
important that CTI have complete and accurate information on this form. This information will be used to
develop the engineering design, CTI's customer specific Internal Specification Sheet, Quality Assurance
requirements (testing and inspection), and certification/documentation requirements of the RFQ and/or
PO.




INSTRUCTIONS:
1)         If you did not enable micros, close and reopen and chose enable micros.

1)         Highlighted areas are essiential to designing and/or quoting the container.

2)         For clarification purposes, there is a definitions sheet which CTI recommends you use if you
           are unclear about any field in the specification.

3)         CTI recommends that you also use the Existing Containers sheet to view containers which are
           already designed and on file at CTI. If you should find that there is an existing container that
           will fit your needs, select it in the drop down menu labeled CONTAINER CHOICE as it will
           save you both time and money. If an existing container is selected, DO NOT TYPE in any of
           the dimension cells or the payload cell, as this would cause you to lose the information
           provided.


4)         Select a container choice. If you do not find an existing container to fit your needs, select
           custom. If you select an existing container, skip to # 7 in the instructions.

5)         Select container type using the drop down menus provided. Be sure to indicate the opening
           style.
6)         Fill in the container dimensions and payload requirements. If you only have either an exterior
           or interior requirement, simply add 10" to all interior dimensions, or subtract 10" from all
           exterior dimensions to complete these fields, then under ADDITIONAL REQUIREMENTS,
           state which set of dimensions are critical.

7)         Select whether an approved design package is required.

8)      For the first 2 sets of specifications, read the CTI STANDARDS, and if you require different
        specifications, type or chose those specifications under SPECIFICATION IF DIFFERENT
        heading.
9)      For the next set of specifications, read the Standard Requirements, and if you require more
        than the requirements listed, state those requirements under ADDITIONAL REQUIREMENTS
        heading for each category. Note that for the DOCUMENTATION REQUIREMENTS, there are
        3 levels of documentation, chose the one that best discribes your requirements, then you may
        add additional requirements


10)     List any additional requirements that were not specified in the previous sections in the
        ADDITIONAL REQUIREMENTS NOT SPECIFIED ABOVE heading. An example of this would
        be the D-ring placement, if you require D-rings.

11)     If you do not find a "This specification is incomplete" statement, then you have successfully
        provided the information required by CTI, and this form can be submitted.

12)     Completed form must have NAME, TITLE, and DATE of approval. You may submit this form
        electronically, by fax, or mail.

 CONTAINER CHOICE: Option to chose an existing container, or chose custom
                   to build your dimension and payload requirements.


CONTAINER TYPE:
                    ST:    Strong Tight(now considered IP1)
                   IP1:    Industrial Packaging Type 1
                   IP2:    Industrial Packaging Type 2
                    7A:    Class 7 Type A
             7A Fissile:   Class 7 Type A Fissile Qualified

         Top Loading: Typical container, access to the inside is through the top
                        of the container
         Side Loading: Also considered a front loading, where access to the
                        inside is through the front. D-rings are typically used to
                        remove and/or install lid.
         End Loading: Container is accessable through an end opening lid.
                        This container is typically used for hinged door
                        applications, for dumping and reusing.
            Cake Box: The cavity of the container lifts off a platter to access the
                        inside of the container. This is typically used when
                        loading heavy machinery.
              Roll-Off: This container is typically used when transporting large
                        quantities of material for dumping and reloading. This
                        container comes with both top and end opening.

    DIMENSIONS:            The minimum interior dimension is the smallest
                           dimensions that you require. The maximum exterior
                           dimensions are the largest outside dimensions that you
                           require, this will determine the burial volume for the
                           container. If you only require one dimension set, add
                           10" to all interior dimensions to get your exterior
                           dimensions, or subtract 10" from all your exterior
                           dimensions to get your interior dimensions, then specify
                           in ADDITIONAL REQUIREMENTS NOT SPECIFIED
                           ABOVE which dimensions are critical.

  DESIGN PACKAGE:          A design package includes a drawing, plus data used to
                           produce the container. Chose yes only if you require
                           these items to be on file, or approved.


      PAYLOAD:             Payload is the amount of weight the container can hold.
                           Note that this does not include the container weight.

      CLOSURE:          Typically a bolted closure is used for all containers,
                        however, for IP1 containers, there is a clip closure
                        option. This specification will determine how the lid is
                        secured to the container.

CLOSURE MATERIALS: The type of closure will determine the closure materials.
                   For bolted closure, bolts must be used, and for clip
                   closure, clips must be used. However, you have the
                   option of materials beyond that. You may specify, for
                   instance, the length and type of bolt used.


       GASKET           This is the sealing material used.

    PRIMER TYPE:        The type of primer used. Note that the primer must be
                        compatible with the paint.

       PRIMER:          Specify where the primer is to be applied. Note that the
                        primer must be applied at least where paint is required.


    FINISH TYPE:        The paint type required for the container. Note that the
                        paint type must be compatible with the primer type.


        FINISH:         This indicates where the paint is to be applied.

   MIL THICKNESS        This applies to the thickness of the paint on the
                        container. CTI's specification is per manufacturers
                        suggestion, however, if you have a requirement for this
                        area, please state that requirement.

       COLOR:           This refers to the color that you require the container to
                        be painted.

  NUCLEAR FILTER:       This is a CTI standard for 7A applications. This filter
                        allows the release of pressure without releasing nuclear
                        waste. CTI's standard for this is a MicroFiltrex
                        #M099421-NPRX. If you require a different nuclear
                        filter, specify that particular nuclear filter in ADDITIONAL
                        SPECIFICATION NOT SPECIFIED ABOVE section.


     RISER TYPE:        Specify the type of risers to be used, 2-way entry or 4-
                        way entry, caps or no caps, and the number of risers
                        required.

  ALLOW STACKING        Specify if you wish to be able to stack the containers.
      DESIGN:           This is applicable if you require a lifting lug, such that the
                        design will allow the lifting lug to set between the risers.

NUMBER OF SECURITY Security holes are not used as bolt holes, although CTI's
     HOLES:        standard is to make the security holes the same size as
                   the bolt holes. This is useful for attaching tags to
                   containers, and typically come in pairs (for example, 1
                   each end).

 TYPE OF MARKINGS:      Specify how you require the container markings to be
                        applied to the container, for example stencil or engraved
                        plate.

  HANDLES/LIFTING       Specify if you require handles or lifting lugs or both for
       LUG:             removing the container lid. Note that lifting lugs are not
                        available for side or end loading applications. Handles
                        are typically used only to align lids for larger containers.


    NUMBER OF           Specify the number of external D-rings required. D-
 EXTERNAL D-RINGS:      rings may be used for tie-downs, or lid lifting
                        applications. Also specify the required D-ring type if
                        specific D-rings are required. Note that for side and end
                        opening containers, D-rings are typically used for
                        removing the container lid.

NUMBER OF INTERIOR Specify the number of interior D-rings required for tie-
    D-RINGS:       downs inside the container. If specific D-rings are
                   required, specify this information as well.

   INSPECTION BY:       Specify if you require someone other than CTI's quality
                        control to inspect the containers.

      MATERIAL:         Specify if you require a particular material thickness for
                        the container panels.

     CONTAINER          Specify if you require additional markings to the CTI
     MARKINGS:          standards. Note that the CTI standards are required by
                        49 CFR.
