  CLEC Verbatim Description of Issue/Action: ANI is missing from the ADUF call records purchased from Bellsouth. In place of the ANI new # assigned,
  this is supposed to be unique to the rate center the call originated in. Numerous instances found where the "BTN" in the from field was not really unique to
  the rate center where that call originated. Jurisdiction missing from the ADUF records. The true jurisdiction from the calls is not available on the record.
  Bellsouth has advised to jurisdictionalize the records based on the # in the from field though that is not a true ANI. Numerous instances found where the
  "BTN" in the from field was not really unique to the rate center where that call really originated. We have over $100k in balances vendors have disputed
  based on the From number in the records not being the true ANI.
  CLEC Comments/Actions Taken/Status/Resolution: 6+ conference calls with Bellsouth have taken place beginning 9/12/07. Disputes have been
  submitted, and resubmitted. AT&T individuals on calls; William Leavy, Steve Trucks, Denise Mitchell, Frank Pagerino, Angie McCurry, Sharon Crocker, David
  Whitney, Betty Steely, Bill McCarty, Kevin Pfeifer.
  CLEC Description of Extent of Impact on Business & CLEC Community: Several other CLECs have also mentioned they are experiencing similar
  problems as a result of the missing ANI and jurisdictional information in the records. These same CLEC’s have asked to be a part of future calls with legacy
  Bellsouth to voice their concerns.
  Supporting Documentation Submitted with Issue: Attached is a document with records we billed a provider, they in turn, came back having matched
  the calls and showed were they were really local. The BTN Bellsouth had in the record was not local to the, to number. But the true ANI was local to the, to
  number.

  07/09/08 CLEC User Forum: This issue has completed the escalation process and is officially denied. There was discussion among Cinergy
  Communications, Time Warner Telecom, Verizon Business, and Logix Communications as to what possible other steps can be taken outside of the CUF arena.
  This issue is moved to the disputed/inactive log.
  06/04/08 CLEC User Forum: AT&T reported that this issue is continuing through the escalation process. No formal response has been made; however, it
  does not look promising. Nuvox Communications stated that the request is not for additional fields, but to receive correct information. Nuvox stated that it
  thought this issue had been taken to the commission since it received a questionnaire. Time Warner Telecom stated that it would refer this to its regulatory
  staff.
  05/07/08 CLEC User Forum: This issue has been escalated to the Executive Director and is pending a formal response.
          ACTION ITEM: In response to Cinergy Communications, AT&T will check to see if the names of the escalation levels can be provided.
  04/09/08 CLEC User Forum: AT&T declines this request due to cost. The switches in the AT&T Southeast Region do not currently record the requested
  information. AT&T would have to upgrade switches, translations systems, and billing system to provide this information. This issue has been escalated at
  Cinergy Communications’ request. There were a number of CLECs attending the meeting that were interested in receiving this information. In response to
  Cinergy’s request for additional information related to the escalation, this issue has been escalated to the fourth level manager, the Executive Director. Also,
  the account management team is aware of this issue.

  03/05/08 CLEC User Forum: This is a new issue that was accepted last week, and has been forwarded to the subject matter expert for evaluation. In
  response to Cinergy Communications, AT&T stated that it hopes to have an update for the April CUF meeting; an internal meeting to discuss this issue is
  scheduled for next week.
  02/27/08 ESC: This issue was discussed and accepted at the February 27th ESC meeting.
