Volunteers assist with therapeutic activities, supportive visits, feeding and early mobilization. Receive training and provide a valuable service. One-year commitment, one 3-4 hour shift each week. NEW HOpE OuTREACH CLINIC COuNSELING Turner Geriatric Clinic social Accessible primary health care services operated as a partnership between New Hope Baptist Church of Ann Arbor and the University of Michigan Health System. A health care team from the Geriatrics Center provides primary care, health screening, health education and social work services for adults age 55 and older. 229 Chapin St., Ann Arbor, 734-998-6085, Monday, 8:30 am12 noon and Friday, 1-5 pm. workers provide counseling services for older adults. Persons who could benefit may be feeling depressed, adjusting to illness, experiencing stress, having marital problems, grieving, or having concern for an elderly parent or loved one. Counselors work with people to set achievable goals and find ways to attain them. Counseling services are covered by most insurance. Co-payments may be covered by secondary insurance. SuBSTANCE ABuSE OuTREACH AND EDuCATION Turner Social Work offers screening, consultation and support to clients and families with questions about how alcohol and/or medications may be affecting their lives. GERIATRICS CENTER CLINICS AND TuRNER GERIATRIC CLINIC For medical appointments call: (734) 764-6831 Blood Pressure Clinic A new clinic is available to provide assistance for older adults with blood pressure that is not wellcontrolled. The Blood Pressure Clinic offers monitoring and counseling by a registered nurse. Testing of home equipment to check for accuracy is also available. The clinic gives additional support for the patient and family as they deal with this chronic disease. The clinic is on Wednesday afternoons. Transitional Care Clinic The Transitional Care Clinic provides assistance to older adults transitioning between the hospital, nursing care facilities, and home. Patients who have been hospitalized or have visited the Emergency Room who require urgent follow-up, as well as patients who were discharged to subacute care or rehabilitation are eligible. The clinic also cares for patients who are in need of short- or long-term nursing home placement to prevent re-hospitalization. Medical care and social work support is provided to patients and families. Increased Clinical Services As of July 2008, the following clinical services have increased appointment availability: Rheumatology, Gastroenterology, Psychiatry and Neurology. Urgent care appointment availability has increased on Mondays and Fridays. Palliative Care Clinic The Palliative Care Clinic provides care to patients with incurable and potentially life-threatening diseases. Palliative care focuses on providing comfort care and good quality of life when a cure is no longer possible. Pain control, symptom management, coordinated and multidisciplinary care, family support and referrals to hospice care are some of the services provided by the Palliative Care Clinic. 3 Meet throughout the year. SILVER CLUB is a program designed to meet the social and activity needs of older adults with memory loss. The group meets six days a week, four at the Turner Senior Resource Center in Ann Arbor and two days at Brown Chapel AME Church in Ypsilanti. The varied activities are based on the interests of current members; these include music of all kinds, drumming, art, photography, gardening, and service projects. EARLY STAGE/MILD MEMORY LOSS PROGRAMS: Coffeehouse: Education and support for people with early memory loss who would like to talk with others and learn about memory loss and strategies for coping. Memory Keepers: Reminiscence group for those who would like to spend time reflecting on and celebrating their past lives. Mind Works: 7-week Brain Gaming Group. One hour of challenging & fun cognitive games to keep the mind active. All Silver Club Programs require an interview or phone reservation; some have waiting lists. All programs have fees, but there is a sliding scale. No one is ever turned away based on income. Services include support for family members. The Osher Lifelong Learning Institute at the University of Michigan will jump-start those gray cells. For 2008-09 you could choose among 43 major lectures, several dozen mini-courses and study groups, Midwest travel to satisfying venues, and our newest activity, “After 5:00,” a series of informal, interactive programs in late afternoons and early evenings.
