Update on Enhanced Perinatal Surveillance (EPS) Project History of EPS • Overall goals: - Monitor implementation of USPHS recommendations on testing pregnant women, ZDV use, trends in HIV among children - Establish surveillance system to respond to Ryan White data requests - Assist in evaluating perinatal prevention efforts • Established in 1999 - 24 areas (16 population-based, 8 facility-based) - Data collected on mother-infant pairs - Multiple data sources EPS Special Surveillance Report • http://www.cdc.gov/hiv/stats/specialreportNumber4.htm • Examples from the report: – Prenatal care of HIV-infected women – Timing of HIV testing of HIV-infected women – Receipt of antiretroviral therapy by HIVinfected women and HIV-exposed infants – Method of delivery for HIV-infected women – Substance use and toxicology screening of HIV-infected women during pregnancy – Screening of HIV-infected women during pregnancy More History • Budget reductions - 10 areas funded in 2004 - 8 areas funded in 2005 - Data for evaluating perinatal prevention efforts What’s next for EPS? • New program announcement for FY 2006 - Approx. $1.8 million - 3 year cycle - Defined protocol and procedures - Revise and provide standard protocol, abstraction form, data collection instruments, and data submission instructions for 2006 Other updates- Pediatric Case Definition • Changes to the CDC Pediatric Surveillance Case Definition – Resolving high number of indeterminates – 32% of infants born in 1999-2001 were classified as indeterminate (EPS Report, 2004) • Clinicians conducting 2nd virologic test on HIV exposed children before age 4 months – Clinically classified as HIV negative – Lost to follow-up – Will remain in indeterminate category as defined in current pediatric case definition Other updates- Pediatric Case Definition – Consultation held April 20-21st 2005 – Recommendations based on consensus – Further research needed on case definition after ARV treatment and timing issues – CDC recommendations forthcoming – Evaluation on high number of indeterminates
