Statement of Purpose Type a short statement indicating why you would like to participate in this program and how it will help you in your life pursuits or career. (500 words max) Resume/CV (2 pages max) listing jobs held, any travel experience, extracurricular activities, and/or any distinctions and honors received. Unofficial college transcripts for all college coursework. Faculty recommendation forms for two (2) faculty members other than the course instructor. Recommendations are to be included with your application in envelopes that have been sealed and signed across the seal by each recommender. Recommendation forms may be downloaded from the course website. Completed applications are to be turned in at the HSU Art Department Office. I understand the following application requirements and course policies:     To be eligible to apply, I must be in good academic standing and have a GPA of 2.5 or above. I understand that I must complete all applicable prerequisites before enrolling or seek instructor approval. Students with any incidence of offensive/disruptive behavior, illegal activities, plagiarism or cheating on file with HSU Student Affairs, at an time prior to or during the course, may not be eligible to apply to or attend the course. Sign: __________________________________________ Date:_______________ Inherent to any study or research abroad program is the stress to student participants caused by unfamiliar surroundings, “culture shock” and the intensity of the program. If one sudent does not act responsibly, is easir is not culturally sensitive, the entire program is negatively affected. Thus in order to enhance the overall operation of any HSU study and research abroad program, as well as each student’s experience, all participating student should be mature, self-motivated, academically competent and ready to accept the challenges of international study, and, if accepted, must attend one of the orientation programs provided by the Study Abroad Office or, if off campus, complete the on-line orientation. For information on dates and times please go to www.humboldt.edu/goabroad A selection process for student participants in HSU International stud and research programs is imperative in order to set and maintain high program standards as well as to ensure the safety for all participants and the continuation of the program year after year. The selection committee will work to select students who demonstrate a solid academic background, a high degree of maturity, and a desire and ability to contribute positively to the study or research abroad program in questions. Greek Art Program, Page 3 APPENDIX F INDIVIDUAL PLEDGE Inherent to any study or research abroad program is the stress to student participants caused by unfamiliar surroundings, culture shock, and the intensity of the program. If one student does not act responsibly, is easily distracted, falls behind in their assignments, or is not culturally sensitive, the entire program is negatively affected. Thus, in order to enhance the overall operation of any HSU study and research abroad program, as well as each student’s experience, all participating students should be mature, self-motivated, academically competent and ready to accept the challenges of international study. The Program Director may send home any individual whose conduct, in the opinion of the Program Director, is detrimental to the program or to the other students. This includes uncooperative or disruptive behavior, alcohol abuse, illegal drug use, and failure to perform satisfactory academic work. Fees will not be refunded if a student is asked to leave the program early. The avoidance of any illegal drug use, drug purchase, or drug sales cannot be stressed strongly enough. Many countries’ officials inflict severe penalties on foreigners breaking domestic laws, especially for illegal drug use. Students are cautioned that a foreign passport and ignorance of local laws will not protect them, nor is it likely that anyone from the program or the U.S. Embassy or Consulate will be able to provide assistance if they are arrested for or convicted of drug use or other crimes. Greek Art Program, Page 7 APPLICATION AND PAYMENT DEADLINES FOR GREEK ART PROGRAM     Application due to HSU Art Department: $500 deposit due: Remaining Program Fees due ($2800*) Deadline for booking airfare February 13, 2009 March 9, 2009 April 13, 2009 April 20, 2009 Students who withdraw prior to April 8th are entitled to a refund of monies paid, less $10 plus any amount already committed or expended on their behalf. Students who withdraw or who are disenrolled from the Greek program on or after April 8th, will not be eligible for a refund. A full refund will be granted if the program is cancelled due to lack of enrollment or other unforeseen circumstance. Students are responsible for arranging their own air travel to Athens and must arrive at the designated place and time. No refunds will be issued until a full accounting of the program costs can be made.
