									               Atlantis Elementary 2nd Annual Talent Show
                     Application and Permission Slip
Fill out the following application if you would like to participate in the Atlantis Elementary Talent
Show on November 13, 2009 at 6:30 pm. This form must be turned in by September 17, 2009.

The student plus a parent or guardian must initial each item below that you understand and
accept the requirements to participate. An application must be filled out by each performer.

_____ _____ My act will not contain any inappropriate behavior.
_____ _____ I promise to respect every student that performs.
_____ _____ My parents and other family members will also respect all of the performers on
             stage.
_____ _____ I, as well as my family, understand that if there is any inappropriate behavior, on or
             off stage, that person will be asked to leave.
_____ _____ If my act is selected, I understand that I will receive an invitation to attend a
             mandatory Parent/Student meeting on Monday, September 21, 2009 in the library.
_____ _____ If I am selected, I will attend a mandatory Rehearsal on Friday, November 6 at
             8:15am and Dress Rehearsal on November 12 at 5:00pm. I will perform my entire act
             for Atlantis Administration and the PTO Board Members, with any/and all props I
             plan to use. If I deviate in any way from my rehearsal on the night of the Talent
             Show, I will be instructed to stop and will have to leave the stage.
_____ _____ I will arrive at the school no later than 6:15 pm on November 13 on the night of the
             Talent Show. I understand that if I am late, I will not be allowed to perform.

These rules are not meant to be discouraging, but rather as a means to ensure that each and
every student has an opportunity to enjoy the night and to do their performance with lots of
praise and encouragement from the audience. We look forward to a wonderful evening!
