					Field Trip Sack Lunch Request Form

Students interested in purchasing a sack lunch from the Food Service Department may do so by
returning this form to their classroom teacher (5) five days before the field trip. Students may
use their lunch tickets or cash to purchase sack lunches. (Sack lunch prices are the same as the
daily lunch prices: $2.75 per student; $0.40 reduced student.)

The Sack Lunch will include your choice of:    (mark your selection below)

□     Peanut Butter & Jelly Pillow Sandwich    OR     □      Grilled Cheese Sandwich

□     Chocolate Milk        OR    □      White Milk
Each lunch will also include:     Fruit Leather, Fresh Fruit, and Chips or Cookie
