Winooski Avenue, PMB 277, Burlington Vermont, 05401 802.862.8360 Form of Registration Under Uniform Gifts to Minors Act The “Uniform Gifts to Minors Act” provides that if the subject of the gift is a security in registered form, by registering it in the name of the donor, another adult person [an adult member of the minor’s family, a guardian of the minor] or a trust company, followed, in substance, by the words: “as custodian for (name of minor) ............ under the [name of enacting state] Uniform Gifts to Minors Act”; In New Jersey and Vermont, the form of registration is the same as above with each adopting State’s name replacing the words within the brackets. Unless otherwise indicated, the italicized bracketed material in the subsection is deleted. * [Note that in Vermont, the subsection as adopted includes the italicized bracketed material.] In South Carolina, subsection 2(a)(1) has been replaced with the following: “as custodian for (name of minor) ............ under the Uniform Gifts to Minors Act”. Form of Registration Under Uniform Transfers to Minors Act The “Uniform Transfers to Minors Act” provides that an uncertificated security or a certificated security in registered form is either: 1) registered in the name of the transferor, an adult other than the transferor, or a trust company, followed in substance by the words: “as custodian for (name of minor) under the [Name of Enacting State] Uniform Transfers to Minors Act”; or 2) delivered if in certificated form, or any document necessary for the transfer of an uncertificated security is delivered, together with any necessary endorsement to an adult other than the transferor or to a trust company as custodian, accompanied by an instrument in substantially the form set forth in subsection (b)” In all States (other than Vermont and South Carolina) the form of registration is the same as above with each State’s name replacing the words within the brackets.
