School Immunization Requirements 2009-2010
The Wisconsin Student Immunization Law delineates the minimum required immunizations for students at each age/grade level. Two different vaccine requirements have been added which may affect your child’s compliance with the law. This change went into effect with the beginning of the 2008-2009 school year. The requirements are as follows:
•Grades K, 1
•Grades 6, 7 •Grades 9, 10 •Grade 12
Varicella (2 doses) Tdap (1 dose) and Varicella (2 doses) Tdap (1 dose) Tdap (1 dose) and Varicella (2 doses)
Varicella vaccine protects against chickenpox. Previously only one dose was required. Tdap is the adolescent vaccine that protects against tetanus, diphtheria and pertussis (whooping cough). What do parents need to do? 1. Have your child vaccinated if he or she has not already received the vaccine(s). 2. Record the date(s) of the immunization(s) in the bold outlined box(s) on the Student Immunization Record, located on the back of this letter. 3. Return this form to your child’s school. Remember it is very important that you always update your child’s school about all vaccines. Exceptions to the Tdap and varicella vaccine requirements. Tdap: If your child has already received a tetanus-containing vaccine (such as Td) within the last 5 years of entering the grade it is required, your child is compliant and Tdap is not required. Check the box marked “Td” on the Student Immunization Record and fill in the date it was received. A history of pertussis disease is not an exception to the Tdap requirement. Varicella: If your child had chickenpox disease or shingles, even after the first dose of varicella vaccine, further doses of vaccine are not required. Check the “yes” box to the chickenpox disease question on the Student Immunization Record. Where can I get Tdap and/or varicella vaccine for my child? These vaccines are available from your child’s health care provider or at your local health department. Summer clinics at local health departments fill up quickly, so please call ahead to ensure your child is up to date before the next school year.
