Requirements: • 18 years of age • Curriculum Vitae or Transcript (for Intermediate and Advanced level students only) • Students applying for Intermediate or Advanced level courses must provide a transcript or certificate demonstrating at least one semester of Culinary Arts course work, or one year of experience in food preparation. Master applicants are required to hold a degree in Hospitality Management and/or demonstrate 3 years of experience in food preparation. STEP 1 Submit application with a non-refundable $80 Application Fee and CV/Transcript. Your CV/Transcript may be sent separately, in which case acceptance will be conditional until it is received. Late applications are considered on a space-available basis and should include payment of the $150 late fee in addition to the Application Fee. STEP 2 (Payment) Notification of acceptance status will be sent to you by email within one week of application. Within 2 weeks of email acceptance the following deposit fees will be due: $300 Security Deposit + 50% of Tuition + Single Room Supplement (if applicable) + $1000 second Semester Tuition deposit (if applicable; for one year visa) This deposit must be paid to SAI in order to secure your enrollment. Acceptance letters will include instructions and payment dates. STEP 3 (Final Payment) Balance of tuition + Student Services and full rent payment and US University credits (if applicable): Fall Semester = July 1 Spring Semester = November 1 Summer = 60 days prior to check-in date Application Deadlines: Fall Semester = July 1 Spring Semester = November 1 Summer = 60 days prior to check-in date *Late Fee: Late applications and late payments will incur a $150 penalty fee. Affiliated Universities: Students from SAI affiliated schools should discuss their plans with the study abroad office at their home school in order to be clear about the payment procedure your school uses in their SAI affiliation. Affiliate students will receive credit for their courses in Italy from their home institution. It is the student's responsibility to insure that credits will transfer. Students from affiliated universities MUST obtain approval from their study abroad office prior to submitting an application. Non-Affiliate Students: All Apicius student will receive a transcript from Apicius. If you do require US credits SAI students enrolling from non-affiliated schools may receive US credit for courses at Apicius through Fairfield University in Connecticut. In such cases an enrollment fee is added to the tuition. It is the student's responsibility to insure that credits will transfer. Student Visas: Instructions for student visas are included in your acceptance letter. For most Italian Consulates, SAI offers a visa processing service for a fee of $250. Students not residing in the US should contact the nearest Italian Consulate, following their directions for obtaining a student visa. For many Italian Consulates, SAI is able to handle the visa processing for you and the fee for this service can be found on the cost page. Students not residing in the US should contact the nearest Italian Consulate, following the directions for obtaining a student visa. Students who are undecided about enrolling in multiple terms should note that it is possible to extend their program after arrival in Florence however they will likely be required to return to their country of residence in order to renew their student visa. Airline Travel: SAI has arranged for students to receive the lowest possible student airfares through Waldorf Travel Service, which can be reached at 800.328.7266. Be sure to tell them you are a Study Abroad Italy student. We strongly recommend finalizing your airline arrangements at least 120 days prior to departure. Insurance: All students, 49 years of age and younger automatically receive full coverage student health insurance while enrolled in their SAI program. Physical Disabilities: Study Abroad Italy will do everything in its power to provide for students with physical disabilities, though it is important to note that Italy’s infrastructure and ability to accommodate disabled individuals differs from that which is available in the USA. Certain sites are better equipped to assist students with disabilities and we recommend speaking with our admissions staff for assistance. Please notify Study Abroad Italy during your application process of any and all disabilities. Financial Aid: Students using financial aid are required to pay Application Fee + Security Deposit + $500 Tuition Deposit + Single Room Supplement (if applicable) + $500 Second Semester Tuition Deposit (if applicable: for one year visa) Confirmation Deposit and Security Deposit. Written verification of financial aid award or loan approval must be sent to our office in order to delay the balance of program fee and before your application is considered complete. If your financial award is not sufficient to cover full Program Fee, the difference must be paid by the final payment date. Please direct all questions concerning financial aid to the SAI California office. Financial Aid students that do not submit all required documents 60 days before program start will automatically be withdrawn. Payment: Checks should be made payable to Study Abroad Italy. All payments, including wire transfers, should list the applicant's name. If you are enrolling for more than one term, a separate application fee must be paid for each term. Payment by credit card may be made on our website. Housing: Housing assignments with address and roommate names will be sent via email no later than 10 days before Check-In. Scholarships: SAI awards a number of tuition scholarships for its programs in Italy. Contact our California office for information. Tuition Refund/Cancellation Policy: Check-in day is considered the start of the term. The Confirmation Deposit is NOT REFUNDABLE. Further Fee liability is based on the date on which a formal cancellation request is received in writing. If a cancellation notice is received: • 60 - 46 days prior to start of term the student is liable for 10% of the full tuition. • 45 - 31 days prior to start of term the student is liable for 20% of the full tuition. • 30 - 8 days prior to start of term the student is liable for 50% of the full tuition. • 7 days or less prior to start of term the student is liable for 100% of the full tuition. PLEASE NOTE: If enrolling for two semester terms, the $1,000 tuition second semester tuition deposit is non-refundable after the first semester application deadline. All other cancellation policies apply. Housing Cancellation Policy: In the case of a student deciding to cancel their housing the following penalty will apply, based on the date of withdrawal: • 60 days or less before the move in date student is liable for 100% of the housing cost. PLEASE NOTE: Failure to attend does not constitute official cancellation. Official withdrawal must be submitted in writing (e.g. e-mail or fax) to SAI California. Failure to meet a deadline or to fulfill a requirement may result in the cancellation or withdrawal of a student’s application. Exceptions are made for financial aid students whose disbursements do not coincide with these deadlines. Please contact SAI California for further instructions.

page 4 Full Name of Student: Full Name of Student's Parent or Legal Guardian: This is a legally binding release made by participant and, if participant is under 18 years of age, by participant's parent or legal guardian. The term "Undersigned" is used in this agreement as pertaining to more than one person. If participant is at least 18 years of age, it refers only to participant. If participant is under 18 years of age, "Undersigned" refers to participant's parent or legal guardian. I. PROGRAM Participant desires to take part in the following program. If participant is under 18 years of age, the Undersigned hereby grants permission for participant to take part in this program: Name of Program: Study Abroad Italy Name of Host Study Abroad Italy Institution: Apicius International School of Hospitality Dates of Program: II. WAIVER This program involves studying at a Study Abroad Italy (SAI) institution in Italy and may include living with a host family or in an apartment in the host city. Group excursions and social/cultural activities are routinely offered to participants. Some trips will be overnight and most will involve transportation on a bus and/or train. The Undersigned fully understands that there are certain dangers, hazards, and risks inherent in international travel, the group excursions, and in the activities included in the Study Abroad Italy program and have signed this document in full recognition and appreciation of the dangers of these activities, which dangers include, but are not limited to, physical injuries (minimal, serious, catastrophic) and/or property loss or damage. The Undersigned expressly acknowledge that the participant is not required to participate in this Study Abroad Italy program, but chooses to do so. The Undersigned therefore agrees to assume and take on all responsibilities in any activities associated with the Study Abroad Italy institution's program in Italy. In consideration of, and in return for, the service, facilities and other assistance provided to participants by the Study Abroad Italy institution or the US office (and their governing boards, employees, and agents) we, along with any of our assignees, heirs, distributees, guardians, and legal representatives release Study Abroad Italy and its Italian institutions from any and all liability, claims and actions that may arise from injury, harm or death to the Undersigned and from loss or damage to the Undersigned's property in connection with these activities. The Undersigned understands that this release covers liability, claims and actions caused entirely or in part by any acts or failure to act by the Study Abroad Italy institution and/or Study Abroad Italy office, including but not limited to negligence, mistake, or failure to supervise by the Study Abroad Italy institution and/or Study Abroad Italy US office or any of their employees, agents, or contractors. III. CONDUCT / BEHAVIOR RELEASE I understand that as a student attending a Study Abroad Italy (SAI) program, I represent my home institution and will behave as an ambassador for that institution. Therefore, all policies governing behavior as printed in my home institution's Student Code of Conduct apply to me during my participation in the SAI program, in addition to the Student Code of Conduct for my chosen SAI program. I understand that information regarding any behavior found disruptive or offensive to the SAI program will be released to my home institution's study abroad office. I undertand that Study Abroad Italy must take steps to ensure that no offensive, disruptive or potentially dangerous conduct occurs. Accordingly, SAI reserves the right to dismiss a student from the program on the basis of any observed conduct or behavior which causes concern for the safety and well-being of students or others. The Director and/or Associate Director of SAI shall have the authority to make the final decision on a participant's dismissal from the program. Misconduct includes actions that violate school regulations or Italian law, or in the judgment of the school and/or SAI officials, jeopardize the welfare of that student, other individuals or the program. It is understood that no refund of tuition, fees or rent will be given if a student is dismissed from the program. Such actions include, but are not limited to, the following: Excessive unauthorized absence from class and/or other organized program activities Violation of Italian laws Damage to or destruction of school, residence, or student property Reckless or dangerous behavior The use of threats or physical violence Violation of the school or housing regulations Alcohol or substance abuse
IV. TRANSCRIPT RELEASE/SECURITY DEPOSIT REFUND Transcripts are released approximately 8 weeks after a term ends. Study Abroad Italy reserves the right to withhold a transcript in the case of unpaid balances due to Study Abroad Italy, its academic institutions or any Study Abroad Italy agent (including Student Cells and housing agencies). Security Deposits and any other monies held will be refunded in full within 60 days of the end of a term minus any apartment damages or unpaid balances due to SAI, its academic institutions or any agent, contractor or program partner. Transcripts will be released once all accounts are paid in full.

Student housing is not equipped with telephones. For security and convenience Study Abroad Italy provides each student with their own cell phone through our partner, Student Cells, Inc. This allows school advisors and family members to contact a student. Before the start of the term, you will receive instructions for picking up your cell phone in Italy. The cell phone is provided at no charge, included in the student services fee and there is no charge for incoming calls while you are in Italy, however, a credit card must be presented when picking up your telephone (Visa or Mastercard only). Any calls placed on the cell phone will be billed to that credit card twice-monthly and will appear as "Student Cells Italy, Sebastopol, CA" on your bill. Please note, transcripts will be withheld for outstanding balances on cell phone accounts and will not be released until accounts are paid in full.

Housing will not be assigned until the program deposit and housing security deposit are received. Housing assignments are made on a first-come, first-served basis, determined by the date deposit is received. Please Note: Single rooms are limited in availability and cannot be guaranteed. There is a supplementary fee for single rooms; check the cost page for information. If you are requesting a single room, payment of the supplementary fee must be received with your tuition deposit (see procedures page Step 2), or you will be placed in a Double Occupancy Room. All students are housed with other SAI students*, most of whom are from the United States. A typical apartment will house 3-8 students, and will contain a combination of single (1 student per room) and double (2 students per room) bedrooms. Some triple (3 students per room) bedrooms may be available. Up to four students will share one bathroom, and all students share the kitchen, sitting area, and washing machine. Rent includes utilities up to $75 (USD) per person, per month. Excessive use may warrant additional charges. Housing assignments are subject to cancellation if full payment of Program Cost is not made by the due date. Please note: full payment of Program Cost is due prior to receiving your housing assignment. SAI will mail one billing statement to your permanent mailing address which will include all due dates.
