2009-2010 KINDERGARTEN REGISTRATION IN THE MORAGA SCHOOLS
Registration of students entering kindergarten for the 2009-2010 school year will be held on Thurs.,
Mar. 5 and Fri., Mar. 6 from 9:00 a.m. to 2:00 p.m. at Los Perales and Rheem Schools, and from
9:00 a.m. to 11:45 a.m./1:15 p.m. to 3:00 p.m. at Camino Pablo School. To register incoming
kindergarten students, parents/guardians must go to the school their child will attend in the
2009-2010 school year.
To be eligible for kindergarten, a child must be 4 years and 9 months of age as of Sept. 1, 2009. This
means that a child must have been born on or before December 2, 2004. It is important that
parents/guardians register every eligible child for kindergarten at this time, as it will be of great
assistance in planning staffing and class sizes for the next school year. Please pass this information on
if you have acquaintances with kindergarten age children who do not have children already in school.
The following process will be followed if enrollment exceeds anticipated capacity: students registered on
Mar. 5 & 6 with siblings currently in school will receive priority; all remaining non-sibling students will be
prioritized by lottery to be considered for redirection to another school in the Moraga School District.
There is no benefit to parents/guardians in registering during any particular time over the two-day
registration period. Should the lottery be necessary, students registered after Mar. 5 & 6 will not be part
of the lottery process and will be redirected.
THE DOCUMENTATION BELOW MUST BE PROVIDED AT REGISTRATION
Registration cannot begin without proper birth and residency verification as noted below!
1. Birth certificate or passport for child (original only).
2. Proof of Residency (originals only): Parent/Guardian must provide one of the following: current driver’s
license, ID, passport or military ID. IN ADDITION: Homeowners must provide two of the following: deed,
utility bill or most recent property tax bill. For those who are renting/leasing: copy of the rental/lease
agreement showing names of all adults/children at residence AND one of the following: utility bill dated within
the last six months or confirmation (on letterhead) from utility company of service in/being changed to your
name (letter should include renter’s name, property address and date service begins). For homes being
built or in escrow: escrow documents showing the closing date, which must be scheduled within two months
(40 school days) of the first day of school; once escrow has closed, two item’s from the Homeowner’s list,
above, must be provided to the school office. If you have alternate living arrangements that do not fit
into the above categories, please contact your school of residency.
3. Child’s social security number (if child has been issued SS#).
4. Verification of the following immunizations: Polio, MMR, DTP, Hepatitis B and Varicella. (See reverse.)
5. Name & address of child’s current preschool.
6. Emergency contact information.
A Report of Health Examination for School Entry form dated on or after Mar. 1, 2009 and an Oral Health
Assessment form dated on or after Sept. 1, 2008 are also required by the Moraga School District. These forms
will be available at registration. Any necessary or updated immunizations completed after registration must be
turned into the school office no later than Aug. 14, 2009. Any child without verified up-to-date immunizations will
be ineligible to attend school.

GUIDE TO THE REQUIREMENTS OF THE CALIFORNIA SCHOOL IMMUNIZATION LAW FOR
Parents of Children In or Entering School or Child Care

WHY YOUR CHILD The California School Immunization Law requires that children be up-to-date on their immunizations (shots)
NEEDS SHOTS to attend school or child care. Diseases like chickenpox, measles, and whooping cough spread quickly, so
children need to be protected before they enter. Most children need booster shots before starting kindergar-
ten. 7th grade entry requirements went into effect July 1, 1999. A varicella (chickenpox) requirement for
kindergarten entry and child care attendance went into effect July 1, 2001.
WHAT YOU WILL You will need your child’s Immunization Record. It must show the date your child was given
NEED AT each required shot. If you do not have an Immunization Record or your child has not received all
REGISTRATION required shots, call your doctor or local health department now for an appointment.

This number includes kindergarten boosters. If your child is 4-6 years old, entry requirements are met with only 3 polio and 4 DTPs if
at least one polio and one DTP dose were after your child’s fourth birthday.

For children 7–17 years old, entry requirements are met with only 3 polio and 3 DTP or DT/Td if at least one polio and DTP or DT/Td
were after your child’s 2nd birthday. For students age 7 years and older, pertussis immunization is not required.

A Td booster is recommended but not required.

One dose must be on or after the 1st birthday regardless of any doses received earlier. The Hib requirement applies only to child care
children under age 4 years and 6 months.

One dose on or after the first birthday is required for grades 1–6 and 8–12. Mumps immunization is not required for students age 7
years and older.

Two doses of the 2-dose hepatitis B vaccine formulation along with provider documentation that the 2-dose hepatitis B vaccine
formulation was used for both doses and both doses were received at age 11–15 years will also fulfill this requirement.

If your child had chickenpox disease, ask your doctor to note it on the immunization record to meet the requirement.

Required for children not enrolled in California schools before July 1, 2001. 1 dose required for grades K–12. For children 13–17 years
old, 2 doses are needed if vaccine received after 13th birthday.
If your child’s record is missing some doses, please contact your doctor or clinic now to obtain the full
immunization record or any doses needed. If your child recently received immunizations and needs an
immunization later in the year, he/she can be allowed to attend, provided you get the remaining doses when
they become due.
Your child may be exempted from some or all immunizations by a doctor because of a medical condition.
Your child may be exempted by you because of your personal or religious beliefs. Ask your school or child
care provider for details.
