DIRECTIONS FOR COMPLETING THE WEEKLY ACTIVITY SHEET
Marketing Internship grades are influenced by how thoroughly and completely you fill out the MARKETING INTERNSHIP WEEKLY ACTIVITY SHEETS. They are due on Mondays (or the first day of the school week) for work completed the previous week. Each sheet can be worth a maximum of twenty five (25) points. One (1) point is deducted for each missing item.
PLEASE PRINT when you are filling the following boxes:
Business Name – official name of the business where you are employed Address – location/mailing address of the business where you are employed Telephone Number – number used to call the business where you are employed Supervisor – name of the person makes your work schedule, assigns your work duties, and does your evaluation Student Name – your first and last name Student ID Number – your school assigned six digit ID number Class Period Number –number of the class period when you have Marketing Credit for Internship – write the amount of credit you expect for this semester of Marketing Internship Wager Per Hour – amount of money your employer pays for each our you work THIS WEEK’S SCHEDULED WORK Date – month & day from the previous week that corresponds to the day listed Start – time when you began your shift IMPORTANT NOTE: Any day you DO NOT work write “OFF” in the START BOX End – time when you ended your shift LAST WEEK’S ACTUAL PAID WORK Date – month & day from the previous week that corresponds to the day listed Start – time when you began your shift IMPORTANT NOTE: Any day you DO NOT work write “OFF” in the START BOX End – time when you ended your shift Hours Worked – total number of hours for each day TOTAL HOURS WORKED LAST WEEK – add each day’s number of hours worked and write that number in the box COMMENTS: – share at least one compliment, reprimand, situations/problems you are dealing with or some tasks completed Feel free to write additional comments on the back of the sheet
