Melody Carter, LAD Dean Metcalfe, LAD Linda Scott, LAD Jamie Robyn, LAD Alan Wayne, POB Clifford Takemoto, POB Mastocytosis Mastocytosis is a disease characterized by the presence of excessive numbers of mast cells in the skin and internal organs, such as the bone marrow, gastrointestinal tract, lymph nodes, liver and spleen • Demographics – Children and adults – M/F equal • Symptoms – Excess of mast cells – Mast cell derived mediators • Histamine, prostaglandins, leukotrienes, cytokines – Hematologic disorders WHO classification of mastocytosis-Adult • Cutaneous mastocytosis • Maculopapular CM • Diffuse CM • Mastocytoma • Systemic indolent mastocytosis (without AHD) • Smoldering systemic mastocytosis • Isolated bone marrow mastocytosis • Systemic mastocytosis with an AHNMD • MDS, MPD, AML, NHL • Systemic aggressive mastocytosis • with eosinophilia • Mast cell leukemia • Aleukemic MCL • Mast cell sarcoma • Extracutaneous mastocytoma WHO classification of mastocytosis-Adult 1 major + 1 minor or 3 minor Major: Characteristic multifocal dense infiltrates of mast cells in bone marrow biopsy (>15 in aggregate) Minor: -Morphology of mast cells: Spindle shaped (>25%) -Detection of a codon 816 c-kit mutation -Flow cytometric co-expression of CD117, CD2 and/or CD25 by the bone marrow mast cell population -Total serum tryptase >20 ng/ml Pediatric-Onset Mastocytosis • Mastocytosis is an unusual disease • Mastocytosis most commonly present before 6 months of age • ~50-60% of cases of mastocytosis are diagnosed in childhood • In children, the skin is the most common organ involved. • Children typically presents with one of three cutaneous disease patterns Aim • Define the characteristics of severe pediatric-onset mastocytosis whose diagnostic parameters may be discordant from classic adult-onset disease criteria. Skin exam: skin biopsy for definitive diagnosis 2. Complete physical exam 3. Serum tryptase, CBC and differential, platelets • Bone marrow considered for: 1.
