Dear Ms Tavegia:

Health Care Excel (HCE) is pleased to submit its proposal pursuant to RFP #: MED-04-015.
This proposal addresses the SURS Analysis and Provider Audits activities under the Professional
Services Component of the RFP. All required content has been included in this transmittal letter.

3.     Bidder's corporate status and relationship to subcontractor. Health Care Excel,
       Incorporated is a 501(c)(3) private not-for-profit company, incorporated in Indiana in
       1974. We have not proposed the services of a subcontractor. HCE will perform one
       hundred percent of the work proposed.

4. Registered to do business in Iowa and corporate charter number. Health Care Excel
   is registered to do business in Iowa and its corporate charter number is W00371191.

5. Bidder's Federal Tax Identification Number. The Federal Employer Identification
   Number is 35-1392974.

6. Compliance with Contract Terms. Health Care Excel will comply with all Contract
   Terms and Conditions as indicated by RFP Section 9.

7. No Inducements. No attempt has been made or will be made by Health Care Excel to
   induce any other person or firm to submit or not to submit a proposal.

8. Non-discrimination. Health Care Excel does not discriminate in its employment
   practices with regard to race, color, religion, age (except as provided by law), sex, marital
   status, political affiliation, national origin, or handicap.

9. Cost and Pricing Information. No cost or pricing information has been included in this
   letter or in the Technical Proposal.

10. Amendments to RFP. HCE has received and reviewed Amendments 1, 2, 3, 4, 5, 6, and
    7 to RFP #: MED-04-015.

11. Prices Quoted in the Proposal.

    a.     The prices quoted have been arrived at independently, without consultation,
           communication, or agreement, as to any matter relating to such prices with any
           other bidder or with any competitor for the purpose of restricting competition; and

    b.     Unless otherwise required by law, the prices quoted have not been knowingly
           disclosed by Health Care Excel prior to award, directly or indirectly, to any other
           bidder or to any competitor.

12. Authorized Signature. The person signing this transmittal letter, Sharon Smith, is
    Health Care Excel's Chief Executive Officer. She is the person responsible for and
    authorized to make decisions regarding prices quoted. She will not participate in any
    action contrary to item 11, above.

13. Subcontractor Statement.       Health Care Excel does not propose the service of a
    subcontractor.

14. Request for Confidential Treatment of Information. There is no request for treatment
    of confidential information included in this proposal.

An original and eight copies of the technical proposal along with a CD have been enclosed. A
sanitized copy of the proposal has not been included, since we are not requesting any treatment
of the information as confidential.
