We’ll primarily focus on the Program Plan
If you wholeheartedly develop your program plan, you can smoothly sail into the Program Review process! So, lets review the Program Plan template and what’s required….
Let’s go through the template…
1st table: numbers of students by disability and then figure the % of your total for each group 2nd table: list services you provide- check ‘em off Explain how you meet Title 5 requirements that your services:

Do not duplicate other campus services Are related to educational limitations of verified disabilities Directly relate to educational participation Promote maximum independence and integration Are consistent with college mission

Enter your DSPS mission statement Attach organizational charts 3rd table: list all DSPS staff members, like so:

Attach course outlines and syllabi Explain process to be sure instructors meet MQs How you keep that info up-to-date
Don’t stop there….
Explain how your goals relate to the college’s unit, strategic or master plan
Student Learning Outcomes
List what your program has developed Report progress and any changes you’ve made as a result of the information you’ve gathered
And don’t forget the budget
Attach a detailed budget summary for the current fiscal year
Last, but not least…Evaluation
Describe activities you use to evaluate efficacy of your program Attach documentation of evaluation activities (questionnaires, annual report, etc.)

Then what happens?????
The System Office has three choices:
The plan is approved
• The plan is incomplete
The plan requires revisions

BEFORE THE PROGRAM REVIEW VISIT, THE SELF-EVAL REPORT IS DUE
The completed Self-Evaluation becomes the basis for the program review site visit. Questions prepared by the site review team will be based on what they have read. Some of the documents they will wish to review are documents you will have mentioned in your reporting.
Self-Eval Report has 3 Sections
Introduction and Overview of each of the categorical programs 2. College-wide Student Services Integration and Coordination 3. Program Specific Self-Evaluation

Once your report is finished, you still have a two day visit!
This is really good news, because you have the opportunity to show them all the stuff you wrote about in your selfevaluation report! Always remember- honesty is the best policy… if you don’t have it, don’t do it or don’t know…just say so!!!
The team that visits is selected by the System Office and will include:
Chief Student Services Officer – CSSO from another college, acting as team leader A System Office Representative At least one representative from each categorical program area: Matriculation EOPS/CARE DSP&S CalWORKS
After they’re gone…
A team report on the visit will be compiled and sent to you within 30 days. If necessary, you’ll have 60 days to respond to the System Office. You’ll be asked to evaluate the review process… don’t hold back … we want this to be the best for everyone!
