above stated amount (tuition and/or fees) in tender of the United States currency. I fully understand and agree that repayment

1.    The maker waives presentment, demand or payment and notice of non-payment, protest and notice of protest.

2.    The maker understands that any delinquent balance not paid by the above due date(s) will be assessed a late charge of
      5% of the total amount due not to exceed $50.00 per month. The penalty for checks returned because of insufficient
      funds is $20.00, each instance.

3.    In case this note is placed in the hands of a licensed collector or any attorney for collection, or suit is instituted hereon,
      the maker promises to pay, in addition to the foregoing, a reasonable collector’s or attorney’s fee.

4.    The maker will inform the institution of any change of address (domicile or place where mail is regularly received)
      which may occur following the signing of this installment plan.
