                                        Transitions Service Plan
                                        (From Integrated Plan)
GOAL 13: To Children and families experience smooth and effective
transitions when entering, moving within, or exiting the program.
OBJECTIVE 1
As children move into the head start and early head start programs, strategies
are in place to ensure smooth and effective transitions for children and their
families.


Activity 1
Selection & Enrollment
To facilitate a smooth transition in the program during the selection and enrollment              1304.41 (c) (1)
process, Family Advocates meet in center with family.

    a. Selection – the Family Advocates mail letters to notify parents of acceptance into
       program.

    b. Enrollment – the Family Advocate makes an appointment to complete
       enrollment paperwork with the family at the center.

Activity 2
Teacher Home Visits

   a. Before Head Start children enter the classroom, classroom teachers visit children
        in their homes. During these visits, teachers become familiar with the
        developmental level of the children and the context of the family.

   a. Welcome to school night, orientation parent meetings are held to invite families            (i)
        to visit the center, meet the teachers and center director ease transition into the
        program

    b. EHS families are encouraged to bring child to the center for a visit(s) prior to           (ii)
       starting in the classroom.

Activity 3
Strategies to ease children into center-based care include but are not limited to the                1304.41 (c) (1)
following:

Infants and Toddlers
Every effort is made to provide secure, predictable, and responsive situations for infants
and toddlers. This is accomplished through continuity and consistency of staff and close
contact with parents, assisting them in becoming advocates in the transition process.                1304.40 (h) (2)
    a. Strategies to support transitions include the use of transitional objects (child

    c. watching the child for cues and temperament,
    d. understanding of the child’s family composition, language, culture, and child

         capture the child’s interest, etc

Preschoolers
Every effort is made to help preschoolers feel comfortable in their new classroom
environment.
 Strategies to accomplish this goal include, for instance, (See You and your three year old
child)

    a. pictures from home displayed on the child’s cubby and

    b. taking learning tools from the classroom to a home visit.

Activity 4 - Children with disabilities

Infant and Toddlers –                                                                                1304.41 (c) (1)
For children co-enrolled with Early Head Start and ECI Programs the family advocate                  (iii)
attends a meeting to develop the IFSP(Individual Family Service Plan.) This meeting
includes the parent, ECI, and EHS staff, teacher & Center Director. The IFSP is shared
with center staff (cook, teacher, driver, etc.) in order to provide appropriately for the            1304.20 (f) (2)
infant or toddler.                                                                                   (iii)

Preschoolers –
When a child is evaluated for a possible disability, the ARD (Admission, Review &                    1304.40(h)(3)(ii)
Dismissal) meeting is attended with parents, center director, teacher, Health Services
Coordinator, and public school teacher. The IEP (individual Education Plan) is
implemented in the Head Start classroom. Whenever possible, public school therapists
(physical therapy, speech therapy, etc.) -services are arranged to be done at the center in
order to reduce unnecessary disruptions for the child.

Objective 2
When children move into a new classroom or a different center in the program,
these transitions will ensure that children feel secure with their new environment.

Activity 1                                                                                        1304.21 (b) (1)
Moving into a new classroom                                                                       (i)
When moving into a new classroom, the following steps are followed:
 a. Currently enrolled children will be reviewed to determine if a child has a need to
     transition classroom to classroom based on the child’s developmental level and the
     age span of the children in the classroom.

  b. EHS children currently enrolled in PEP sites will be reviewed to determine if any
     child is ready to transition to the community site.

Infant and toddlers
All centers have a small number of children and all children are known by all teachers.
One month before moving into a new classroom, the child visits the new classroom with             1304.21 (a) (3)
their primary care teacher for one hour of activities each day for two weeks.                     (ii)

When possible:
a. Two weeks before moving into the new classroom, the child visits the classroom for
  two hours of activities (including mealtime). The current teacher stays with the child
  for one hour and go back to her classroom for the second hour.

b. One week before moving to the new class, the child is in the new classroom for 4
  hours of activities. The current teacher stays for one hour and leaves the child for the
  remainder of the time in the new classroom.

c. Any time children are exhibiting anxiousness or distress, they returned to their base
  of support. During these transition activities, accommodations are made in order to
  ensure that child/staff ratios and group size are not compromised.

Activity 2
Moving from one site into a new site, the following steps are followed:

As Early Head Start children transfer from one site to another, see the Procedures to
transition Early Head Start Children Throughout the year, children and families may
move within our service area, Early Head Start to Early Head Start transition may occur

Records are transferred to new site with the child.

When possible:
a. Before moving into the new classroom, the child and parent visit the classroom. The
   parent stays with the child for a period of time and then goes back to home
   classroom.

b. During the first week before moving to the new class, the child is in the new
   classroom for activities. The parent and child may stay for period of time and then
   leaves the child for the remainder of the time in the new classroom.

c. Any time children are exhibiting anxiousness or distress, the parent maybe contacted
   for support. During these transition activities, accommodations are made in order to
   ensure that child/staff ratios and group size are not compromised.

Throughout the year, children and families may move within our service area Head Start

As children move out of the head start and early head start programs,
strategies are in place to ensure smooth and effective transitions for children
and their families.

Infant and Toddlers
For all children in Early Head Start six months before their third birthday, written
transition plans are developed. These plans specify the needs of each child in relation to
ongoing services beyond Early Head Start.

The plan might include transitioning to Head Start per Early Head Start Transition to
Head Start Procedure, or transitioning to another early childhood program. The family
advocate coordinates this information with the center director.

Includes health, development, progress made, family circumstances, availability of
Head Start or other services CDCs ( See You and your three year old child)

EHS children may remain several months until transition into Head Start. Per Early
Head Start Transition to Head Start procedure.

Activity 2 –
Preschoolers

    a. Center Directors arrange to have the principal from the local elementary school
       attend a parent meeting at the end of the school year to acquaint parents with
       expectations and policies of kindergarten classrooms.                                 1304.40 (h) (2)

    b. At the end of the school year, three & four year olds visit the pre-kindergarten      1304.40(h)(3)(ii)
       /kindergarten programs at the public school they will attend at the start of the      1308.21 (c)
       new school year, when ever possible.

    c. Parents are invited to visit the school and complete public school registration       1304.40 (h) (2),

    d. Information and relevant records are shared with the parent’s permission with
       the kindergarten school personnel as needed and appropriate.

Moving On Celebration - At the end of the school year, children and parents are              (ii)
recognized for participation and involvement in the program throughout the year at each
center. All children receive a certificate, regardless of their parent’s level of
participation.
