About Your Fracture Fee
                       “GLOBAL SERVICE PACKAGE”

Our office uses a standard billing process referred to as a “global fee” for
fractures (broken bones). This fracture fee is normally charged upon initial
evaluation by the doctor and covers a variety of services included in a
“global service package”. The following is an explanation of this “global
service package”:

Your fracture fee covers the doctor's history and physical examination, x-
ray interpretation, application and removal of the initial cast and all follow-
up visits up to 90 days from the time of the fracture. Follow-up x-rays and
any additional casting supplies are not included in the global billing fee.

Physical therapy charges are not included in the “global service package”
and will be billed separately from the fracture fee.

We will bill your insurance company directly or send a claim form to you for
your submission to the appropriate party. If you do not have health
insurance, a payment plan will be arranged.
