    HOW TO FILE YOUR MARYLAND UNEMPLOYMENT INSURANCE CLAIM USING THE
                                INTERNET

The Department of Labor, Licensing, and Regulation, Division of Unemployment Insurance offers the
option of filing your unemployment insurance claim over the internet instead of contacting a Claim Center
by phone.

YOU CANNOT FILE OVER THE INTERNET IF:
  You do not live in Maryland (unless wages earned during the last 18 months were from a work location
     in Maryland).
  You have not earned wages from a worksite in Maryland during the last 18 months.
  You worked for the Federal Government in the last 18 months.
  You have worked for more than 3 employers in the last 18 months.
  You have filed for unemployment insurance in another state in the last 18 months.
If you do not meet any of these requirements it will be necessary for you to contact a Claim Center by phone
to file your claim. A list of telephone numbers is on the back of this sheet.

YOU WILL NEED THE FOLLOWING INFORMATION TO FILE OVER THE INTERNET:
 Your social security number (SSN) and mailing address.
 A telephone number where you can be reached.
 The name, date of birth, and SSN of all dependents under age 16.
 If you are not a citizen, proof of your alien status.
 The business name, address, and dates of employment for all employers within the last 18 months.
 If you seek work through a union with a hiring hall, the hiring hall number.
 If you were in the military within the last 18 months, your DD214, Member 4.

HOW TO FILE YOUR CLAIM:
 If you meet all of the requirements listed above, you can file your claim by going to our website at
  www.mdunemployment.com
 In the “Claimant Information” area of the website, click on “File an Initial Unemployment Insurance
  Claim”.
 At the bottom of the Welcome Page, click on “Begin Filing Your Initial Claim”.

You will then be guided through each area of the application. It is important to read all instructions
provided. At the end of the application you will be given your rights and responsibilities prior to submitting
the claim. It is critical that you read all of the information provided in the application to prevent a
delay or possible denial of benefits.

Once you have submitted the claim you will be given a confirmation number. You should print or write
down your confirmation number. You will also have an opportunity to print a summary of all information
you provided as well as your rights and responsibilities. After your claim is filed, you will receive
information in the mail in two to five days.
