To report the annual security statistics and developments in 2005/2006.

Summary:

This report outlines the security activities for the financial year 2005/06, with particular regard to the
requirements of the NHS Security Management Service.

                                       Annual Security Report 2005/2006

1.0   INTRODUCTION

1.1   This report for Brighton & Sussex University Hospitals NHS Trusts covers the period 1 April 2005 to 31
      March 2006.

1.2   In line with the NHS Security Management Service strategy document “Protecting your NHS: A
      Professional Approach to Managing Security in the NHS” this report will refer to the 7 generic actions
      for managing security:

      Creating a pro-security culture
      Deterring those who may be minded to breach security
      Preventing security breaches from occurring
      Detecting security incidents and breaches and ensuring these are reported
      Investigating security incidents and breaches
      Applying sanctions against those who breach security
      Seeking redress through the criminal and civil justice systems

      And the four priority actions for the protection of the following:

      Patients and staff
      NHS Property and assets
      Maternity and paediatric units
      Drugs, prescription forms and hazardous materials

1.3   The overall number of all reported physical assaults has decreased from 125* in 2004/05 to 80 this year.
      The overall reported number of non-physical assaults has decreased from 1020 last year to 707 this year.
      A large proportion of this reduction is due to transferring wards at Brighton General Hospital to South
      Downs Health NHS Trust. (protection of patients and staff).

1.4   Reported losses of NHS property has decreased from 60 in 2004/05 to 31 in 2005/06. The number of
      burglaries has decreased from 21 to 18 (protection of NHS Property and assets)

1.5   Reported loss of staff and patient property has decreased in 2005/06 to 124 compared to 140 in 2004/05.

2.0   CREATING A PRO-SECURITY CULTURE

2.1   Trust Induction

      A total of 1009 new staff undertook the 1-day Trust Induction last year. The security department has a 40
      minute session on the Trust Induction. New staff are given an overview of security problems within the
      Trust, a very brief introduction to conflict resolution and an overview of the resources invested by the
      Trust to date.

2.2   Risk Management Committee and Health and Safety Committee

      The Trust Security Manager now attends both committees and is able to raise security management
      issues. It is planned for both committees to take a far greater role in scrutinizing the security
      arrangements within the Trust.

      The Trust Security Manager will be ensuring regular reports are tabled in respect of security objectives
      and progress against national targets.

2.3   Staff awareness

      Staff are generally good at reporting concerns but more needs to be done to raise awareness. Last year
      staff reported 62 incidents of suspicious persons on all premises to the security department.

2.4   Annual Security Report

      All NHS bodies are required to ensure that their Security Management Specialist produces an Annual
      Security Report which should cover the work carried out in the seven generic and four specific areas of
      action. This Trust, and its predecessor, Brighton Health Care NHS Trust has been producing Annual
      Security Reports since 1993. This report is the second that refers to the seven generic areas of action.

      It is recommended that this report should be formally copied to Divisional Directors, Associate Directors,
      Heads of Department and Managers following consideration by the Trust Board.

3.0   DETERRING SECURITY INCIDENTS OR BREACHES

3.1   Development of physical security measures across the Trust’s hospitals, eg. through provision of CCTV,
      access control, visible intruder alarm, asset tagging and presence of security personnel is variable.
      Unsurprisingly, such measures are most developed at the Royal Sussex County Hospital.

3.2   The Trust has, for a number of years, publicised its commitment to providing a secure environment
      through on-site posters and signage. Signage advertising the use of CCTV is used extensively across the
      main hospital sites. Further signage prohibiting the consumption of alcohol in the grounds of the RSCH
      has been in place for a number of years. Posters are also displayed warning potential offenders of the
      risks of assaulting Trust staff.

4.0   PREVENTING SECURITY INCIDENTS OR BREACHES

4.1   Sharing best practice

      The Trust’s Security Managers regularly meet and communicate with security managers and security
      advisors at other health bodies to share best practice and, where appropriate, share intelligence on
      security breaches.

4.2   Conflict Resolution Training

      Between April 2004 and March 2008, all front line staff in the NHS are required to complete a one day
      training module on conflict resolution, based in the National Syllabus. With a workforce of just over
      5000, the majority of whom are frontline this represents a significant challenge to the Trust. Since April
      2004 only 201 staff have been trained in conflict resolution, despite 2 courses per month at both the
      RSCH and PRH being organised. The training is delivered by Roy Bright and Glenn Clark, Security
      Operational Managers for the RSCH and PRH respectively and takes a significant amount of time away
      from their primary duties of managing the operational security teams and investigating physical assaults
      on staff.

      The number of staff attending conflict resolution training is still low. When compared to the total number
      of staff within the Trust, it only represents 2.3% of the workforce trained in dealing with aggression.
      Medical staff are still not receiving any training in this subject.

      Training is mandatory for all front line staff and other staff identified as being at risk based on risk
      assessments carried out by ward and departmental managers. The Security Department is able to assist
      where appropriate with the completion of risk assessments.

      The one-day course designed to prevent violence from occurring, but it is a fact of life that conflict
      resolution training will not resolve all situations and assault may still occur. The next logical step for staff
      is to attend “breakaway training” which allows them to disengage safely from an attacker and make their

      escape to safety. Breakaway training is only provided for staff who have completed the conflict
      resolution training. The security department is able to provide breakaway training to staff.

      In November 2005, the SMS announced that by 31 March 2006 252,000 frontline NHS staff in England
      will have received this training, of which 201 staff have been trained within the Trust. This represents
      0.07% of all frontline staff trained in England. On current training schedules, the Trust will not meet the
      SMS requirement to train all frontline staff by 31 March 2008, in deed it would take 54 years to train
      5,000 of our existing staff at the current rate.

      Unless more training capacity is created, including trainers, venues and ensuring staff are released for
      training, the Trust will not meet the requirement to train all front-line staff.

      In 2005/06 the Security Department provided conflict resolution training to 42 General Practitioners and
      100 staff from Adur Arun & Worthing PCT and Mid Sussex PCT.

4.3   Use of technology

      As well as deterring incidents and breaches, the use of technology such as CCTV and access control does
      help to prevent security incidents and breaches. Whilst the number of CCTV cameras has significantly
      increased at the RSCH, the number of security staff to monitor and respond to incidents has decreased.
      This means that the camera system is not being worked to its full capacity to detect and to respond to
      crime, nor can best evidence be gathered if the control room is unmanned.

      In terms of access control, the majority of wards and departments still rely on electronic or mechanical
      keypads on doors. Such technology is relatively low-grade and easy to compromise. The Trust-wide
      access control system is being installed across the Trust as funding becomes available. CCTV and access
      control is included in all new capital projects at the planning stage. There are currently 171 doors, gates
      and barriers on the access control system with 218 swipecard readers controlling access and egress.
      There are currently another 18 doors with 29 card readers waiting to be commissioned. The access
      control system is managed to allow authorized staff only to use specific card readers; not all staff are
      given access to all readers unless their job requires this level of access. This means that there are large
      numbers of access control profiles set up for individual buildings and departments. For example, the
      Audrey Emerton Building has 9 access control profiles, Sussex House has 12 and RSCH Theatres has 8.
      In order to ensure the integrity of the access control system and the areas it protects full time management
      of the system is required, but not yet established. The new Childrens Hospital will be the single largest
      project to date for introducing access control to a single building with over 350 RACH staff, an as yet
      undetermined number of RSCH-based staff, 80 doors and 160 readers to set up into access control
      profiles.

5.0   DETECTING SECURITY INCIDENTS AND BREACHES AND ENSURING THESE ARE
      REPORTED

5.1   As the statistics show, the reporting of incidents and security breaches over the fours years has remained
      steady. The reporting of incidents and breaches is to be encouraged as it allows the Trust to identify areas
      of weakness and take measures, where appropriate, to reduce or eliminate risk. However, it is noted that
      not all security incidents or breaches are followed up with an investigation due to limited resources.

5.2   It is vital that staff report any untoward, potentially violent or violent incident to the RSCH Security
      Control Room on extension 7474 or the PRH Security Office on extension 4444. Security staff will then
      attend as appropriate and/or call for police assistance. Staff are encouraged to call Security sooner rather
      than later.

6.0   INVESTIGATING SECURITY INCIDENTS AND BREACHES

6.1   In general, staff are good at reporting incidents and breaches. Given the large number of reported
      security incidents and breaches it is impossible to investigate each report. Priority is given to
      investigating the most serious incidents; however, this only represents a very small proportion of the total.
      This means there is a real risk that the Trust remains exposed to recurrences of similar types on incidents,
      i.e. the Trust fails to learn the lessons of security breaches and incidents.

6.2   Where a member of staff is suspected of committing a criminal offence and the allegation is being
      investigated internally, the interview must be conducted by two fully trained and accredited persons, such
      as Accredited Security Management Specialists. Witness and suspect interviews must be conducted in
      accordance with the rules of the Police And Criminal Evidence Act using the standard investigative
      interviewing model. This requirement has not yet been worked through to dovetail with the Trust’s
      Disciplinary Policy.

6.3   Security Managers have carried out a number of internal investigations concerning alleged incidents of
      criminal actions in 2005/06.

6.4   A requirement of Secretary of States Directions is for Accredited Security Management Specialists to
      investigate each physical and non-physical assault on staff: since there were nearly 800 reported
      incidents last year, this has simply not been possible given the current workloads for the Trust’s security
      management specialists.

7.0   SEEKING TO APPLY SANCTIONS

7.1   Currently, when suspected offenders are apprehended by staff (normally security officers) the police are
      called, who attend and then if appropriate arrest the individual.

7.2   The table below shows all offences for which the Trust have been kept informed by the Police.

8.0   SUPPLYING INFORMATION SO THAT REDRESS CAN BE SOUGHT

8.1   Security incidents and breaches have a direct impact on the resources allocated to the NHS to deliver high
      quality patient care. Time and money spent on replacing stolen NHS equipment, repairing and replacing
      damaged NHS equipment or dealing with the consequences of violence is time and money diverted from
      the delivery of health care.

8.2   Where losses are incurred the Trust should seek redress from those responsible for causing them.

8.3   There are two principles behind effective recovery:

      Monies lost through security related incidents and breaches can be returned to patient care.
      Recovery delivers an important deterrent message to staff, patients and visitors that crime does not pay
      and that NHS will pursue redress from those who attack it and deprive it of valuable resources.

      Historically, the Trust has sought compensation through the courts for incidents of criminal damage,
      however more work needs to be undertaken to ensure redress can be sought whenever appropriate.

9.0   PROTECTION OF NHS PROPERTY AND ASSETS

9.1   Despite the physical security measures in place throughout the Trust the emphasis must still be placed on
      the human element, i.e. personal responsibility, to ensure that property and belongings are
      secured.

9.2   There were 21 reported incidents of burglary and attempted burglary within the Trust showing no change
      from last year.

9.3   There were 31 reported losses of Trust property compared to 60 the previous year.

9.4   In the vast majority of most thefts the perpetrators did not need to break and enter the premises.
      Invariably, the thefts tended to be of an opportunist nature. The number of insecurities is a major concern
      as it presents more opportunities for the criminals to enter our premises with the intention of stealing,
      causing criminal damage, or worse still, assaulting our staff and patients.

9.5   Whilst the number of burglaries and property is relatively small, it should be noted that there were 104
      detected incidents of Trust buildings or departments being found insecure outside of working hours. This
      represents 104 opportunities for thieves to steal or damage valuable Trust equipment and premises, which
      could potentially delay or stop patients from receiving timely and high quality clinical care. It should also
      be remembered that many of these insecurities leave staff and patients vulnerable to assault and other
      crimes.

9.6    Security staff responded to 18 intruder alarm activations within BSUH, compared to 19 last year. This is
       a relatively low number however there are a significant number of departments and buildings within the
       Trust that do not have any intruder alarm systems. This is a continuing area of concern. An audit of
       departments at the RSCH reveal that approximately 50% do not have any intruder alarm system installed.

9.7    Photo I.D badge/access control

       Staff I.D. badges are issued with a printed expiry date and the access control system is programmed to
       terminate access rights to departments and buildings on that expiry date. The system is being reviewed to
       highlight to staff their responsibility to replace their I.D. card before the expiry date is due.

       The number of Trust buildings and departments having the access control system installed is increasing
       and will continue to increase. The resources required to update, maintain and develop the access control
       system needs to be increased in order to ensure the integrity of the system and maximize the protection of
       our premises, staff and patients. In particular there is a requirement to ensure routine audits of access
       rights to individual areas and liaise with the 33 other organisations which work on our sites. This
       resource is currently not available.

       At the RSCH the photo I.D. suite has moved from Level 5 to main reception. This allowed the space on
       level 5 to be used to become the hub room for the CCTV and security systems. It also transferred
       responsibility for producing badges from the security officers, who were frequently unavailable, to the
       receptionists. This has resulted in providing a better service to staff when they need to obtain an I.D.
       badge.

10.0   PROTECTION OF MATERNITY AND PAEDIATRIC UNITS

10.1   There has been no new work undertaken in the last year in this field, however it should be noted that the
       new Childrens hospital has been designed to include CCTV, access control and alarm systems. These
       systems will be linked to the Security Control Room. The new Childrens Hospital will have 32 static and
       Pan, tilt & zoom (PTZ) CCTV cameras and over 80 doors with access control. This building alone will
       increase the number of swipecard readers at the RSCH and PRH by 68%.

10.2   It is recognised that the Childrens Hospital will be relocated from its current location on Dyke Road to
       the RSCH in 2007; however the lack of dedicated security personnel presence on this site, along with its
       dated CCTV and limited access control is still a cause for concern with regard to the safety of staff and
       patients. The children’s hospital has to rely on security officers attending incidents from the RSCH,
       which then depletes the security resources at that hospital.

10.3   There is a need to increase security staffing levels at the RSCH in order to cope with protecting patients
       and staff when the new childrens hospital opens at the RSCH in April 2007.

11.0   DRUGS, PRESCRIPTION FORMS AND HAZARDOUS MATERIALS

11.1   There has been no new work with regard to reviewing security of drugs and prescriptions this year.

11.2   Inspections of two areas within the Trust were carried by Sussex Police under the remit of Anti
       Terrorism, Crime and Security Act 2001. A number of recommendations were made in both areas to
       improve securities, work is currently underway in one area, but there are a number of other areas still
       awaiting improvements. The Trust is due for follow-up inspections to establish what improvements have
       been made.

11.3   Sussex Police are also currently auditing other areas within the Trust under the remit of Anti Terrorism,
       Crime and Security Act 2001.

12.0   SECURITY OF PERSONAL PROPERTY

12.1   The number of reported thefts of staff’s property has decreased from 61 last year to 48 this year.

12.2   Despite this low number of losses it is regrettable to say that the majority of thefts that did occur could
       have almost been certainly prevented if simple precautions had been taken by the victims, i.e. keeping
       possessions on one’s person or locking valuables and cash away.

13.0   SECURITY OF PATIENTS’ PROPERTY

13.1   The loss of patient property has decreased this year. There have been 76 losses this year compared to 79
       the previous year.

13.2   The very nature of hospitals means that wards and departments cannot be secured 24 hours a day without
       severely impacting on the level of care that is provided to in- and out-patients. Thieves do enter hospitals
       in order to steal money and valuables if the opportunity presents itself. Staff, patients and visitors need to
       remain vigilant at all times. In-patients must continue to be encouraged to leave valuables at home or to
       hand them over to ward staff for safekeeping.

14.0   MOTOR VEHICLES

14.1   There have been 5 reported incidents of thefts of, or from, motor vehicles from Trust property this year,
       compared to 9 last year. There was also the theft of 2 bicycles from the RSCH. These are very low
       figures given the huge number of vehicles that enter our hospitals each year.

       The PEAT Inspection Team assessed each hospital site for its security measures. Scores are shown

16.0   THE SECURITY SERVICES DEPARTMENT

16.1   Staffing

       The Security Department currently comprises the Trust Security Manager, 1 Security Operational
       Manager at the RSCH (Roy Bright) and Security Operational Manager at the PRH (Glenn Clark), and 1
       Security Support Services Manager (Wendy Burley). A further two security officer posts were lost at the
       RSCH in year. This means that, at best, there are only 3 security officers on duty for 25% of the time at
       the RSCH. These 3 officers are required to man the control room, provide patrols of the RSCH campus
       (particularly in A & E), BGH and the RACH, respond to incidents and assist with security investigations.

       There are now 16 security officers at Brighton and 8 security officers at the Princess Royal Hospital
       providing a 24 hour service across the Trust.

16.2   On 20 March 2006, the Trust ceased providing security services to all other NHS Trusts and PCTs. This
       was due to the Trust’s security officers not requiring a license to operate as in-house operatives but would
       require a license to provide services under contract, as required by the Security Industry Authority.

18.0   KEY OBJECTIVES 2006/7

18.1   To work towards compliance with Secretary of State’s directions regarding security management in
       accordance with the following: Secretary of State’s directions to tackle violence against staff; a
       professional approach to managing security in the NHS; Secretary of State’s directions on NHS Security
       Management measures

18.2   To continue investigating all physical assaults on staff in accordance with Secretary of States Directions.

18.3   To recruit additional security officers in preparation for the post-commissioning of the new Childrens
       Hospital.

18.4   To work towards Safer Car Park status at the Princess Royal Hospital.

18.5   To manage the security programme for the commissioning of the new childrens hospital.

18.6   To survey buildings and departments requirements for required physical security measures so that a
       formal programme of works can be developed to start from 1 April 2007. The Capital Projects

        Development Review Group have formally approved a significant level of funding for the years 2007/8
        and 2008/9 to improve the security of “at risk” buildings and departments

        Whilst it is to be commended that the expansion of security systems is continuing to improve the safety of
        staff, patients and property, it must be highlighted that such systems require on-going monitoring,
        management and administration in order to their realise their full potential and to maximize the return on
        the significant investment made by the Trust. The use of security systems does not replace security staff or
        procedures, but compliments them.

        Funding for an initial appointment of a dedicated CCTV Operator (37.5 hours a week) for the RSCH has
        been secured. The purpose of this is to establish whether it is feasible to split the operational security
        tasking to dedicated CCTV Operators and dedicated security officers, as these roles have now developed

        into two separate professions for which different skill sets are required. If successful it is planned to
        extend the number of dedicated CCTV Operators for each control room.

        In addition, many of the requirements of Secretary of State Directions are new and represent substantial
        increases in the workloads for the security department.

        Regrettably, the number of security officers, security management specialists, conflict resolution trainers
        and administrative support has not kept pace with the above developments.
18.11   Security Control Rooms

        It is planned to provide a new security control room in the main entrance area of the PRH. This will
        allow for the development of CCTV and access control at the PRH and will allow the security service to
        development along similar lines to that at the RSCH.

        It is essential that a control room is commissioned at the PRH this year as it will provide the central
        location for the monitoring and management of the security systems which are being installed, i.e. CCTV,
        access control, video entry systems, intruder and personal attack alarms.

        Whilst not strictly within the reporting year, the new Security Control Room has been commissioned at
        the Royal Sussex County Hospital. This provides an appropriate working environment for the security
        staff and allows for future expansion of the CCTV system. The CCTV system has been upgraded and is
        significantly more user-friendly for security staff and has considerably better recording facilities which
        will provide even better evidence to pursue prosecutions. The Trust has invested £300,000.00 in the new
        control room.

18.12   Conflict Resolution Training

        All courses for 2006 at the RSCH and PRH are fully booked. It is intended to provide a further 15 days
        training for all RACH staff prior to them moving to the new Childrens Hospital. The security team are
        reviewing alternative provision of additional training sessions.

19.0    CONCLUSION

19.1    The reduction in the continuing rise in the number of violent incidents remains the highest priority. Whilst
        it is encouraging to note that there has been a reduction in reported violence this year it is too early to
        establish whether this is the start of a longer-term trend.

19.2    It is essential that there is investment in training of staff in conflict resolution and breakaway training plus
        providing additional trained and qualified security staff at the RACH to deter violent and non-violent
        crime and to respond to violent incidents that cannot be resolved by nursing staff.

19.3    Overall activity for the Security Department continues to be extremely busy.

19.4    The Security Management Specialists will continue to develop work in the seven generic areas for
        improving security, particularly with regard to the investigation of assaults on staff.

19.5   The provision of CCTV and access control continues, but these improvements will be hampered by the
       lack of a dedicated security control room at the PRH.

19.6   It is a matter of urgency that the number of CCTV cameras is increased and the lighting levels improved
       to provide enhanced security of people and vehicles in the grounds of PRH.

19.7   Significant investment in physical security measures are still required to protect Trust premises and
       property from loss due to theft and criminal damage, particularly with regard to high value medical
       equipment and I.T. equipment: a programme of significant investment will commence from April 2007.
       This is over and above continued investment made by individual departments or as part of capital project
       schemes.

19.8   The Trust is continuing to make progress in improving security but there is still a huge amount of work to
       undertake to improve security at a faster rate than that which is currently being achieved.

19.9   The continued development and success of the integrated access control system will be determined by
       ensuring an adequate level of management of the system, as the system is expanded to cover more wards,
       departments and buildings.
