                       Women’s Lacrosse Player Registration & Member Form
The Woodstock Women’s Lacrosse Booster Club has been developed in order to successfully maintain and have a Lacrosse
Program at Woodstock High School. To be part of the Women’s Varsity Team and Lacrosse Club you must complete the form
below and return it along with your payment of $450.00 to the address below no later than February 1st , after February 1st
there will be a $45.00 late fee. A payment plan is available with the approval from the President and Executive Committee and
must be proposed in writing. Payments received by December 1st will receive a 10% discount. Any student who does not make
the team or cannot play due to grade eligibility will be refunded their payment. NO REFUNDS after season starts. If you

Lacrosse Player Fees are $450.00 per player. The fees paid are used to pay for the Women’s Lacrosse Program, all away
Game Transportation, Field Maintenance, Field Equipment, Referees and Officials, Gate Fees, Tournaments, Practice & Game
Uniforms, Facility Costs, Medical Supplies, Coaches Salaries, School Insurance, Liability Costs & County Fees. In order to
maintain and participate in this program everyone must submit payment no later than February 1st.
