To obtain a new passport, you will need the following items:
Completed Form DS-11, Application for a U.S. Passport ,
Note: Do not sign the form until requested to do so by an authorized agent.
Evidence of U.S. Citizenship (one of the following)
o o o o o Previously issued, undamaged U.S. Passport Certified birth certificate issu by the city, county or state issued Consular Report of Birth Abroad or Certification of Birth Naturalization Certificate Certificate of Citizenship
Identification (one of the following)
