JOB DESCRIPTION: An individual who practices podiatric medicine which involves the diagnosis, medical, physical or
surgical treatment of the ailments of the human foot with the exception of administrations of general anesthetics and the
amputation of the foot.

REQUIREMENTS:
       Age:                         21

          Education/Experience:     a. Graduate of an approved college of podiatric medicine and has attained the academic
                                       degree of Doctor of Podiatric Medicine (D.P.M.).
                                    b. Proof of completion of one year post graduate residency program approved by the
                                       Council on Podiatric Medical Education.

          Examination:               a. National Board of Podiatric Medical Examiners Exam (NBPME) - Parts I and II
                                     b. Illinois Written Clinical Competency Examination (PMLexis).
                                        Exam given by: Continental Testing Services, Inc.; 547 South LaGrange Road;
                                        LaGrange, IL 60525; Phone: (708) 354-9911.

                                     June and December - Illinois Written Clinical Competency Examination

PERIOD OF ISSUANCE:                 Two years (All licenses expire on January 31 of each odd numbered year.)

RECIPROCITY/ENDORSEMENT:           Yes:
       a. Must be at least 21 years of age and of good moral character.
       b. Requirements for licensure in the state of original licensure were at that time substantially equivalent to the
          requirements then in effect in Illinois.
       c. Pass the Illinois Written Clinical Competency Exam or request a waiver. Consideration of a waiver requires
          submitting one of the following for review:
                1) Proof of certification by American Specialty Board.
                2) Proof of achievement of special honors or awards.
                3) Proof of publication of articles in recognized and reputable journals.
                4) Proof that applicant has written or participated in the writing of textbooks in podiatric medicine.
                5) Proof of successful completion of a written clinical competency examination taken in another state.

        NOTE: Licensure requirements, licensure fees, examination fees and exam administration are subject to change.
