The CNIB Library VISUNET CANADA Partners Program Application for CNIB Library Service is multi-purpose.
Partner library staff can use this form: to register a blind or visually impaired patron for CNIB Library service; to register a non-CNIB print-disabled patron on their institutional record (a sub-record); to create or modify a patron’s profile [the profile indicates the patron’s choice of format(s), frequency of delivery, and subject preferences for reading materials]; to add or delete magazine subscriptions in DAISY format; to register the patron for access to The CNIB Digital Library or Children’s Discovery Portal;
to register the patron for VisuNEWS by phone.

CNIB LIBRARY VISUNET CANADA PARTNERS PROGRAM APPLICATION FOR CNIB LIBRARY SERVICE BACKGROUND NOTES ON THE VISUNET CANADA PARTNERS PROGRAM Please read the following information before completing the CNIB Library Service Application Form, including the Subject Preferences Form, attached. The numbers below refer to the same numbered questions on the application form. 2. SELECT ONE OF THE FOLLOWING DESCRIPTIONS: The Partner library must identify the patron as someone with a visual impairment or someone who has a print disability not related to vision, for example, a learning disability or physical disability that prevents the patron from holding a book. This selection will determine the type of patron record to be created. A person who has a print disability, but is not visually impaired, will have an institutional sub-record created -- for example, Bob Smith, c/o Winnipeg Public Library. The institution’s contact information will be recorded in the registration record. 4. PATRON INFORMATION Note on disability selection: This information confirms that the Partner library has deemed the patron eligible for service under the Canadian Copyright Act. 8. MATERIAL FORMATS REQUESTED BY THE PATRON DAISY talking books (DC) - These digital audio books on CD require a special digital playback machine or computer software. See Section 7 of the Manual for more information on acquiring DAISY software or players. One version of the software is free and available through the CNIB Library’s Web site at: http://www.cnib.ca/library/digital/demo/index.htm Talking books on 4-track cassettes (RC) require a 4-track Talking Book Machine. Please note as of July 2004 the CNIB Library is no longer 8-2
producing new titles for our circulating collection in 4-track format. New audio titles will only be available in DAISY format. Descriptive videos (DV) are VHS video cassettes of films and PBS television programs with narration added between dialogue sequences which describes the visual elements of the film / program, such as facial expressions, costumes, settings, actions, etc. An Audio cinema (AC) cassette is a classic film on 2-track cassette with added narration describing the visual elements of the film. Braille books (BR) are available in contracted or uncontracted braille. Uncontracted braille is essentially the braille alphabet; one braille symbol equals one letter. In contracted braille, one symbol may represent a letter, a sound or a word, depending on its placement in the sentence. Most braille readers require contracted braille. PrintBraille (PB) materials are children’s picture books with inserted transparent sheets of braille; the braille corresponds to the printed word. This format enables a blind person and a sighted person to read together. Note on entering format and frequency of service for the patron: Indicate how many items should be selected by the CNIB Library and how often (i.e., weekly or monthly). Typically, adult patrons request books on a weekly, bi-weekly or monthly basis. Example: _X__DAISY bks: __2_ per ___X__ week or _______ month __X_Descriptive videos: (number) __1_ per ____ week or __X_ month This means the patron would like to receive two DAISY books per week and 1 Descriptive video per month. If the patron wants to receive titles by request only it isn’t necessary to choose subject interest codes (Question N. 11 on the form). However, it is important to complete Question N. 8 for these patrons since the system needs to refer to a profile indicating the kind of format(s) the patron will receive, as well as a frequency (eg. 1 item per week) so the system will check regularly for items the patron has requested. (See Section 2-7 of the Manual for more details about loan periods for various formats.) 8-3
Note for Descriptive videos and Audio cinema: These are a random circulation of film titles not based on any subject preference. The age selection on the application form will determine the age level of the video sent to the patron or Partner library. Note for unacceptable categories for reading preference: Patrons who receive automated profile delivery need to have some control over their book selection, just as a patron who browses the book stacks in a public library would have control over choices. Feedback from patrons has indicated that there are aspects of the reading experience they do not want. For example, a patron who is hard-of-hearing may find it problematic to listen to the higher pitch of a female voice or a narrator with a British accent. Patrons also need to decide, for example, if they would prefer not to receive books containing strong language. This category can be sensitively discussed with each patron. 10. OTHER SERVICES THE CNIB DIGITAL LIBRARY AND CHILDREN’S DISCOVERY PORTAL The CNIB Digital Library offers password access to print-disabled library patrons to an online repository of digital information such as online digital audio books, e-texts, electronic braille books, Web sites, newspapers, and access to online databases. Patrons access The CNIB Digital Library using access technology such as a screen reader with synthetic speech or refreshable braille display or screen magnification software. If the Partner library notes the patron’s desire to be signed up for The CNIB Digital Library service here, along with the patron’s e-mail address, a Reader Advisor will send an e-mail to the Partner library patron which will include their individual user name and initial password, as well as an electronic version of The CNIB Digital Library manual. 5. TYPE OF LIBRARY SERVICE The CNIB Library offers two types of automated service. Please select either one or both of the following: ____1. By request by library patron or Partner library. Only items that have a hold placed on them will be delivered. [NOTE: If the patron selects option 1 only, then please skip to MATERIAL FORMATS REQUESTED BY THE PATRON section]. ____2. Automated delivery based on a profile indicating reading preferences. For option 2, the CNIB Library’s system selects books on behalf of the patron based on the patron’s reading interests. PROFILE EXCEPTIONS Please note that most popular literature contains some aspect of violence (e.g., murder mysteries), sex (e.g., love stories) and strong language (e.g., detective fiction). reduction of books available to the patron. Select categories only if the patron strongly agrees that they do not want to receive books in those categories.
