The specified DBS/POSB Credit Card (“Card”) must be valid for at least 3 months from the date of this application. Should you cancel or lose your Card, please make alternative payment arrangements to the respective organisations. Successful enrolment is subject to approval from respective organisations. If the payment requested is not approved, please make other payment arrangements with the respective organisations. Please allow up to six weeks for the processing of your enrolment. Please continue to pay your bills to the various organisations until you see the amount reflected on your Card monthly statement. Please contact your respective organisations should you wish to terminate this payment arrangement.

In the event of any change in your Card number including a replacement Card, you will need to notify the various organisations to continue this payment arrangement. Payments that are charged to your Card exceeding its credit limit available at the time of debit will be rejected. Please ensure that your credit limit is sufficient for these payments. You warrant that the above information provided is true and correct and authorise DBS Bank to verify the information in this form. Should there be any changes in your personal details provided above, please ensure that you update the various organisations. If your existing account is paid by GIRO, the GIRO payment will be terminated. DBS Bank Ltd and the respective organisations reserve the right to amend these Terms & Conditions without prior notice and reject or decline any application and terminate any payment arrangement in our sole discretion without assigning any reason whatsoever.

Excludes: Prushield policies, US Dollar policies, single premium policies, recurring single premium policies, top-up premiums and policies purchased under CPF Investment Scheme and Supplementary Retirement Scheme. Family members: Spouse, children, parents, parent-in-laws, brothers and sisters.
