                                        INTRODUCTION


This resource directory has been developed to provide information on the Exceptional Family
Member Program (EFMP) and the services available in the Central Savannah River Area
(CSRA) surrounding the Fort Gordon area.

The EFMP is a Department of the Army program designed to assist family members who have
special needs. This is accomplished by assigning the military sponsor to installations where the
applicable services are available. Enrollment in the EFMP is mandatory for all active duty
soldiers.

The EFMP is a multidisciplinary program consisting of medical, educational, and social service
professionals. The Army Community Service (ACS) role with the EFMP is to provide information
about the program, assistance with enrollment procedures, advocacy for client services and
provide contact with other community services.

Hopefully, the information provided within the resource directory will assist you in finding
community resources that may be used in addition to the services available on the installation.

SUMMARY OF SERVICES: Our services are developed to enhance the lives of our older
population and include the following:

Planning: The Area Agency on Aging engages in research and formulates comprehensive plans
which identify the social and economic concerns of older adults and the resources available to
meet their needs.

Advocacy: The purpose of advocacy is to develop a responsive awareness of the identified
needs and concerns of the elderly within the community.

Program Development: The Area Agency on Aging selects and supports projects which
successfully overcome individual and social barriers to economic and personal independence for
older persons.

Funding: The Area Agency on Aging receives funding for special services from a number of
sources. The Agency also administers funds that are provided, in part,
by Title III of the Older American Act.

ELIGIBILTY: Anyone who is a bereaved patient

SUMMARY OF SERVICES: A self-help organization offering friendship and understanding to
bereaved parents. The objective is to promote and aid parents in the positive resolution of the
grief experienced upon the death of their child. Every meeting we divide into small sharing

groups. Some meetings, a film is shown, or have a speaker, or some other program before
going into our sharing groups. We also have a sibling support group for bereaved siblings, teen-
aged and older. This Group meets the same night as the parent support group.

ELIGIBILITY: Specific to particular program

SUMMARY OF SERVICES: Inpatient and outpatient rehabilitation for stroke, traumatic head
injury, chronic pain management, neuromuscular/demyelinating diseases, multiple orthopedic
injuries, spinal injury, arthritis, amputee and other disabling illnesses and injuries.
Multidisciplinary
services include the following: physical medicine, physical therapy, occupational therapy,
speech therapy/audiology, cognitive retraining, personal care services, pastoral care, vocational
re-habilitation, social work services, psychology, neuropsychology, nursing, neurodiagnostics and
recreational therapy.

Walton Rehabilitation Hospital is a 50 – bed, not-for profit facility, sponsored by University and
St. Joseph Hospitals. As a comprehensive rehabilitation facility, WRH provides both inpatient
and outpatient services to residents in Augusta and the surrounding communities (90 -mile
radius) who are recovering from physical Disabilities.
