The complete notarized application form, download from website or phone the office. 2. The jurisprudence exam - an open book exam based on the OT Act and rules, available online 3. Fees: payment may be by personal check, cashier’s check or money order, in US funds, through a US bank made out to TBOTE. Application fees are not refundable. We charge a fee for returned checks. LICENSURE BY EXAM You must successfully pass the NBCOT exam in order to obtain a Texas license. Visit NBCOT’s website at www.nbcot.org to register online or phone NBCOT for their candidate handbook at 301/990-7979. We retain exam scores for one year. If you took the exam within one year of applying and had the scores sent to Texas, they will be on file. Those who pass the exam must apply for a license, as their certification is not a license. Texas law requires applicants to obtain a license within two (2) years of passing the exam. Foreign-trained applicants must consult with NBCOT, as they will determine the eligibility to sit for the exam. TEMPORARY LICENSURE A temporary license is only available for an applicant never licensed and taking the exam for the first time. Requirements for a Temporary License: • Requirements 1-3 listed above under general requirements, plus: • Signed Verification of Supervision form, available to download from on our website. • NBCOT’s Confirmation of Examination Registration and Eligibility to Examine form mailed to the Board. This form reports in which 90-day window you will take the exam. Should you re-schedule, you will have voided the terms of your temporary license and it will be revoked. • Additional fee for the temporary license The holder of a temporary license must work under continuing supervision (OT Rules, Chapter 373) of an occupational therapy practitioner until the regular license is in hand. TBOTE converts the temporary license to a regular license when we receive the passing score. Failing the NBCOT will result in revocation of the temporary license. Failure to have scores sent to TBOTE will result in a VOIDED Temporary License. Read Chapter 364 in the OT Rules for further clarification or contact the Board. LICENSURE BY ENDORSEMENT If you have already taken and passed the NBCOT exam and hold a license in another US state, District of Columbia, or US Territory, follow the instructions above, plus: • Contact NBCOT to send us a score report and • Contact your state board to send us verification of your current license. Other Information Should your address or name change during the application process, send us your change of address and a copy of any name change documentation. Incorrect addresses or name discrepancies delay licenses.
