MONITOR’S INSTRUCTIONS
This bulletin is a reminder to staff of a useful, but under-used and mostly forgotten form. The DCFS 5120, Monitor’s Instructions for Face-to-Face Visits and Telephone Calls, is an agreement signed by the monitor that (s)he will follow the department’s instructions. Instructions on how to use the form are in Procedural Guide 0400-504.45, Monitored Visits/Contacts. The form is now available on LAKids and as an attachment to the Procedural Guide. A thank you to the Forms Work Group who brought to the attention of the Policy Section the usefulness of this form and recommended the release of this FYI.
