                      RESERVED FUNDING APPLICATION PACKET


Instructions
Please read the following instructions and all sections of the application form before beginning.

Both these instructions and the application form are available in Adobe PDF format on our website at:
www.nefa.org/grantprog/americanmasterpieces/tourappguidelines.html.

1.   Submit a fully completed copy of the application form. A separate application form must be submitted for each
     artist/company for whom funding is being requested. When determining your budget, round all figures to the nearest dollar
     (please do not include cents). Prorate for the project or artist/group for which the funding is being requested. Do not combine
     figures for series/festival events.

     The New England Foundation for the Arts (NEFA) is participating in a national effort to increase public arts agencies’
     accountability to the legislators and Congress which appropriate the funds given to presenters as grant awards. For this reason,
     the grant application forms require you to provide your U.S. Congressional District number. To find your Congressional
     district, refer to either of the following links: www.visi.com/juan/congress; www.vote-smart.org, or contact your town or
     county clerk's office, your local Board of Elections or Canvasses, or the League of Women Voters. NEFA cannot process
     applications that do not provide accurate legislative information.

2.   Answer the Narrative Questions and attach your answers to your application. The Narrative Questions page may be
     duplicated, or answers may be pasted on in the interest of space and legibility.

3.   Submit a copy of the contract or Letter of Agreement, dated and signed by representatives of both the presenting
     organization and the artist/performing group (if it has been finalized by this time). If the contract is not yet finalized,
     please wait and return the signed contract with the evaluation form following the engagement.

4.   Submit a copy of your tax-exempt determination letter. Our funder The National Endowment for the Arts, has asked that we
     have a tax-exempt determination letter on file for all grantees. We can only make grant payments to nonprofit entities.

5.   Submit a copy of you IRS W-9 form. The IRS requires that we request this from all of our grantees.

6.   Be certain to keep one copy of all documents for your files.

***Mail one copy of the program application form, the answers to the narrative questions, a copy of the signed contract, and
your tax-exempt determination letter to NEFA (address above) to the attention of “AMD Program Associate” for RECEIPT
by 5:00 p.m. EST on April 1, 2009. Late or incomplete applications, as well as applications received by fax or email, will not
be accepted. NEFA is not responsible for applications lost in transit.

You will receive confirmation of your grant in May 2009.

Required funding credit
The National Endowment for the Arts (NEA) and NEFA require that appropriate credit appear in all programs, advertising,
publicity, and promotional materials. Failure to credit the NEA, the American Masterpieces: Dance (AMD) program and
NEFA will result in cancellation of your grant.

REQUIRED CREDIT LINE:

The following credit line must be included, along with the National Endowment for the Arts (NEA) and New England Foundation
for the Arts (NEFA) logos, in all printed and web-based materials related to this project:

“(Name of dance work) was made possible by the National Endowment for the Arts’ American Masterpieces: Dance initiative,
administered by the New England Foundation for the Arts”

                  •   Information regarding the NEA logo can be found on their website www.nea.gov at
                      www.arts.gov/national/masterpieces/logo.html.
                  •   In addition to the credit line above, the NEFA logo should be included whenever possible in all materials.
                      The instructions for use, which were included in your initial award letter, are the following: You can
                      download web and print-ready versions and usage guidelines at www.nefa.org/about/logo.html. The logo is
                      meant to enhance (not replace) the credit language specified in this letter. When possible, please use both
                      the credit language and the NEFA logo. If you have a website, you may credit NEFA by placing the NEFA
                      logo and a link to NEFA on your website in addition to crediting the NEA by placing the NEA and AMD logo
                      and a link to the NEA on your website.

Changes in project or artistic fee
Approval of changes in a project’s scope or artistic fee must be requested in writing at least one month in advance of the
engagement. If there is a reduction in the artistic fee, NEFA withholds the right to reduce the grant proportionately.
Notification of changes in dates or cancellation of the project must be made in writing. Cancelled events will not be funded.

Final reporting and grant payments
AMD touring grants are paid on a reimbursement basis. Along with the grant award letter, you will receive information on how to
access the final evaluation packet from our website. The form and support materials must be completed and returned within three
weeks of the performance/residency’s completion date. A check will be issued within three weeks of receipt by NEFA of these
reporting materials. NEFA will mail checks through the U.S. postal service unless otherwise requested by the presenter. If an
alternate means of shipping is requested, the presenter must pay any associated costs.

If a special need arises, a presenting organization may request an advance grant payment of up to 75% of the grant awarded.
These requests must be submitted in writing (in the form of a letter) to Vanessa Sweeney, AMD Program Associate, at least one
month in advance of the project’s start date. Such requests will be considered on an individual basis and are at the discretion of
NEFA. A copy of the contract signed by both parties must be on file or sent with the request.

NEFA reserves the right to cancel a grant or demand reimbursement from a presenter if that presenter cancels a contracted date
with an artist without adequate notice to the artist and NEFA, or if the presenter employs unprofessional and/or unethical
practices in negotiating terms with an artist.

Note: With your Final Evaluation, you will be required to submit a completed Receipt Form acknowledging your payment
to the artist(s). The form must be signed by an authorized representative of the artist(s) and by the authorized
representative of your organization. This final evaluation and evaluation form will be available on our website at:
