INSTRUCTIONS FOR FACULTY EVALUATION Go to web site: www.pubinfo.vcu.edu/inmadm/faceval1.asp Click on: Faculty Member – Update or print your own evaluation Click in the box and input the first four letters of your last name and the last 4 digits of your social security. Click on: Complete your evaluation form EXPECTED EFFORT Click in each box and type the expected effort for this calendar year. Do not use text, but type a number. SCHOLARSHIP Scroll down to the number of papers published in peer and non-peer reviewed journals this year and click in that block and type number. You may cut and paste from your CV the list of publications and input in List Publications for Calendar Year. If you chose to you may just type this information in the block. Put cursor in block for innovative project and type names of projects that was conducted this year. Put cursor in block for research goals and type goals for the upcoming calendar year. Department and division chair will record research rating. CLINICAL Note: The goals you input last year are listed in expectation block. Place cursor in block for clinical goals and type goals for this calendar year. Department and division chair will record clinical rating. EDUCATION Note: The goals you input last year are listed in expectation block. Go to block across from teaching award(s) and type yes only if you received a teaching award during the evaluation year, otherwise type nothing. Place cursor in block under title of teaching award(s) and type title of award(s). Note: Previous Teaching Awards will be listed if you input before. Place cursor in block under leadership position(s) held in a national or regional organization and type in this information if it applies. Place cursor in block across number of lectures/professorships and type in the number for the evaluated year. Place cursor in block under educational goals and type goals for this calendar year. Division and Department chair will review and edit if necessary. If there are plans for promotion and/or tenure, place cursor in this block and type this information here. Place cursor in mentor block and type the name of your mentor if it applies. Department and division chair will record teaching rating. ADMINISTRATION Note: The goals you input last year are listed in expectation block. If faculty member has any VCUHS/SOM committee membership place cursor in this block and type the name of committee(s). If there is any Administrative Salary Support place cursor in this block and type requested information. Place cursor in block under administrative goals and type goals for this year. Department and division chair will review and edit if necessary. Department and division chair will record administrative rating. SUMMARY Department and division chair inputs overall rating here. Place cursor under block marked Comments – Faculty Member and type overall comments of the evaluation. If you have any involvement on external study for grants (federal and/or non-federal) you may enter it here. Division chair will record his/her overall comments of the evaluation. Department chair will record his overall comments of the evaluation. SUBMISSION After you have reviewed the form, click the “submit” button. At this time the division chair will be able to open this document to record comments. After the division chair review he/she will click the “submit” button and the department chair can access the file for review and comments. The administrative office will then print the form. The department chair and division chair will meet to discuss each evaluation. The evaluation will then be returned to the faculty member, division chair, and department chair for signature.
