Instructions for Participating in the 2006-2007 Louisiana Textbook Adoption Cycle

1. Read the enclosed Information Sheet carefully. 2. Intent to Participate Form: a. This form should be completed and signed by an authorized officer/agent of the company. b. Each publishing company should submit only one original form. Do not submit copies or multiple forms. c. Identify and include the names of those individuals who are authorized to represent your company in business matters in Louisiana. d. Mail the original, completed form (with the participation fee*) to the address as shown. The deadline for receipt is 4:30 p.m. Friday, February 10, 2006. 3. The $100.00 participation fee, in the form of a company check or money order, should be made payable to the Louisiana Department of Education. No personal checks will be accepted. This participation fee must be sent (with the Intent to Participate Form) to the State Textbooks Office by 4:30 p.m. on Friday, February 10, 2006.
*Note: This year, this $100 fee will be waived for any publisher who paid last year. However, all companies wishing to participate must submit the Intent to Participate Form and attend the Mandatory Publishers’ Orientation.
4. Each publishing company must have a representative present during the mandatory Publishers’ Orientation on Friday, February 17, 2006, from 1:00 to 4:00 p.m. in the Louisiana Purchase Room (1-100) on the first floor of the Claiborne Building at 1201 North Third Street in Baton Rouge, Louisiana. 5. After orientation, detailed instructions, rules, and procedures will be presented during the Publishers’ Orientation meeting and required documents (bid sheets and correlation documents) will be available via email and/or on the Department of Education’s website.
