Write your name, the name of team you are evaluating, and the names of the team members you are evaluating in the spaces provided. Write your comments in the appropriate category boxes. Always keep your comments impersonal. For example, don’t single out individual team members. Look for the positive aspects of project – you will always find some! Word your comments such that they are helpful. For example, if you liked the way a team’s mindmap was organized, back up your comments with concrete examples. One-word comments such as, “Great” and “Horrible” are not appropriate.
