SUPPLEMENTAL L ONG TERM DISABILITY INSURANCE P LAN
You can supplement your base group disability benefit with additional individual, non-cancelable, guaranteed renewable
coverage that can replace more of your income if you become disabled.
You are eligible while you are a member of the House Officers Program and you are actively at work. Actively employed on a
full-time basis is defined as not losing work due to a condition which would preclude the ability to perform one’s normal
$1,000 per month or $12,000 per year.
180 Days – commonly referred to as “waiting period.”
To age 65.
DEFINITION OF DISABILITY
Provides 100% of your monthly benefit, up to your maximum benefit period, if you are unable to perform the material and
substantial duties of your own occupation, not working in another occupation and under a physician’s care (this requirement
can be waived with written proof acceptable to Unum, if it is of no further benefit).
RESIDUAL D ISABILITY BENEFIT (PROPORTIONATE TO LOSS )
Your benefits for less-than-total disability are a percentage of your loss from prior earnings, payable for your benefit period.
CATASTROPHIC BENEFIT (1)
Provides a $4,000 monthly benefit in addition to your base policy in the event of certain very serious disabilities that are likely
to increase your living expenses. Benefits are paid if you lose the ability to perform two or more Activities of Daily Living
(eating, bathing, dressing, transferring, toileting and continence); become cognitively impaired; or presumptively disabled.
MENTAL DISORDER COVERAGE
Pays benefits for the duration of your benefit period if you are disabled due to dementia resulting from a stroke, trauma,
infections or degenerative disease (such as Alzheimer’s). For all other mental disorders, pays benefits for disabilities related
to mental, nervous or substance abuse for a total of 24 months during the life of the policy.
Because the program is sponsored by the Henry Ford Health System, all House Officers enrolled receive a lifetime discount.
NON-CANCELABLE UNISEX PRICING
Disability insurance premiums are rapidly increasing, especially for physicians and women. The costs associated with this
plan have been negotiated on a unisex and non-invasive physician rate basis. The following rates are level, guaranteed
not to increase once issued, to age 65.
