Instructions for completing MVA Application
You will need the insurance card for your vehicle and the current MVA Registration Certificate in order to complete the application. If the MVA Registration Certificate lists a
co‐owner, both the owner and the co‐owner must sign the application.
On the right side of the application, complete only the top two sections. Under “Organization Member or Medal Recipient,” check the box that indicates whether it is the
owner or the co‐owner who is a member of the Bryn Mawr Community. Under “Check Class,” check the box indicating whether the vehicle is a passenger car or a multipurpose
vehicle (SUV, van or truck). Ignore all other sections on the right side of the application.
You must turn in your old plates to the MVA within 30 days of receiving your new plates. Please mail your completed application and a check for $50 made payable to the
BMS Alumnae Association to:

Note: The expiration date for your vehicle’s registration will remain the same (i.e., your new BMS plates will come with stickers for the same month and year you have
