Kaiser Permanente offers several proven programs to help smokers quit at no extra
charge for CalPERS members. Programs offered include Quit Tobacco workshops,
Freedom from Tobacco series, and Mom and Me Smoke-free sessions. Participation in a
Kaiser Permanente Quit Tobacco class or counseling program qualifies members to
obtain a prescription for tobacco cessation medication that is covered under the
prescription benefit. The prescription co-payments for Kaiser Permanente HMO members
are $5 generic/$15 brand, up to a 100-day supply. In addition, Kaiser Permanente offers
an online HealthMedia® Breathe™ program at https://kp.org/healthylifestyles or go to
https://kp.org/quitsmoking. When surveyed by Kaiser Permanente, 58% of those who
completed this online program said they quit smoking.

Blue Shield of California
Blue Shield offers members Healthy Lifestyle Rewards, an online, interactive and
confidential program. The program provides valuable tools to help you quit smoking,
and offers cash rewards up to $200. Sign up for Healthy Lifestyle Rewards and get $50
when you complete the online Wellness Assessment. Earn an additional $50 for each 12-
week period of program participation ($150 for 36 weeks). The Healthy Lifestyle
Rewards program is available to Blue Shield of California (CalPERS) members over the
age of 18. This program is not offered to Blue Shield members who are enrolled in the
65 Plus (HMO).

Smoking cessation drugs are covered for members after completion of a smoking
cessation class, or program. This benefit is limited to one course of treatment per calendar
year. Members may contact their primary care physician for information regarding these
classes. Co-payments for Blue Shield Access +HMO, EPO and NetValue HMO members
are $5 for generic brands, $15 for brand formulary, $45 for non-formulary, and $30 for
medically approved and prior authorized non-formulary drugs.

California Correctional Peace Officers Association (CCPOA) - Blue
Shield
Blue Shield covers smoking cessation drugs for California Correctional Peace Officers
Association (CCPOA) after completion of smoking cessation classes or programs. This
benefit is limited to one 12-week course of treatment per calendar year. Members may
contact their medical group or IPA for information about these classes and programs.
Blue Shield will reimburse the cost of the drugs only, minus the copayment, after
receiving a copy of a certificate of completion for a smoking cessation class or program.
Participants are responsible for the cost of the smoking cessation class or program.

Blue Cross offers members access to its smoking cessation program, The Last Cigarette.
The Last Cigarette offers numerous resources and tools to assist you to quit smoking.
The free kit includes Cold Turkey Quitting Calendar Magnet; TLC Core Quit Kit; Stop
Smoking Personal Action Guide Booklet; National Quitline Flyer and Precision Rx Order
Form. In addition, Blue Cross will provide its members up to $100 for counseling and
alternative treatments.

California Association of Highway Patrol (CAHP) - Anthem Blue Cross
Behavior modification programs utilize methods to identify your smoking habit and to
modify behavior to successfully quit smoking. Behavior modification does not consist of
hypnosis, shock therapy, acupressure, acupuncture or other similar methods to alter
behavior.

The Plan will provide 100% reimbursement up to $100 payment for one approved
behavior modification smoking cessation program per Member during the Contract Year.
Benefits are limited to twice per lifetime for each Member.

Benefits are provided when verification of completion of one of the following approved
programs is submitted to Anthem Blue Cross:

   •   American Lung Association - "Freedom From Smoking." Call 1-800-586-4872 or
       your local lung association office or visit the Web site at www.lungusa.org for
       information.
   •   Medical clinic or Hospital-based programs. Consult your family Physician or
       local community Hospital for information.
Many individuals find the use of nicotine replacement therapies helpful with their attempt
to combat nicotine addiction. However, the use of most of these products has only been
shown to be successful when used in conjunction with a smoking cessation program
which works with individuals to understand their smoking habits. The combination of a
nicotine patch or a prescription medication such as Zyban or Chantix, to help alleviate
withdrawal symptoms, with a smoking cessation program, to assist with the behavioral
aspect of the smoking habit, and the desire to quit generally leads to success.

When you successfully complete one of the approved smoking cessation programs and
submit a Certificate of Completion, the Plan will provide 50% reimbursement up to $175
payment for either a 90-day supply of nicotine patches or a 90-day supply of the
prescription medication Zyban or a 90-day supply of the prescription medication Chantix
per Member during the Contract Year. Benefits are limited to twice per lifetime for each
Member.

Benefits are provided for covered expense incurred up to a maximum of $100 per lifetime
for approved behavior modifying smoking cessation programs. Behavior modification
does not consist of hypnosis, shock therapy, acupressure, acupuncture or similar methods
to alter behavior. Benefits are provided when verification of completion of one of the

Self Help Program: The Smokenders program is a 7-week audio self help program that is
available only to Members who live beyond 25 miles from approved class-supported
program locations or who work shifts that are not compatible with class-supported
programs. Anthem Blue Cross has negotiated a significant discount for Smokenders kits,
which must be obtained by requesting a special coupon. To determine the Member's
eligibility for the Smokenders program and to obtain a Smokenders coupon, call the
PORAC-Anthem Blue Cross customer service unit.

Note: Smokenders programs purchased from any other source will not be reimbursed.

Benefits will be provided subject to the following:
     1.     The Member must enroll in an approved Smoking Cessation Program and
         retain the payment receipt.
     2.     The Member must request a Health Promotion Program Reimbursement
         Form and a Certificate of Completion from the PORAC-Anthem Blue Cross
         customer service unit.
     3.     The Member must obtain the instructor's signature on the Certificate of
         Completion, verifying that he or she has completed the program, attended every
         session and that the Member is smoke free at the time of the program's
         completion.
     4.     The Member must mail a copy of the signed Certificate of Completion and
         Reimbursement Form with the receipt to Anthem Blue Cross for
         reimbursement.

No co-pay except any amount in excess of covered expense.

Prescription smoking cessation products, such as Zyban and Chantix, are available at the
standard Medco retail or mail pharmacy co-payments. Note that coverage limits may
apply. Over-the-counter smoking cessation products are also available at a discount at the
retail or mail pharmacy provided you have a prescription.
