					                   Regional Advisory Council Annual Report
                                      Report Form
 An annual report is to be submitted to DSHS, OEMS/TS in or around October 15th , and
 will report on/for the past fiscal year (September 1 thru August 31) in accordance with
Texas Administrative Code, Title 25; Rule 157.123: Essential Criteria II. D. 4. Additional
         information can be entered or submitted as an attachment to this report.

1. On a separate form (Attachment A) provide current information for RAC Officers and
   Executive Committee/Board as of September 1st.

                                               Needs are collected annually in March and are
                                               reviewed for common needs. Any common
                                               needs are brought forward to the appropriate
                                               standing committee for discussion and
                                               recommendation to the Board of Directors.
                                               Additionally, the standing committees review
                                               regional issues that may have arose during the
                                               year and make recommendations to the Board.
                                               We have been very fortunate to be able to
                                               provide our members with education and lots
                                               of equipment and supplies.

3. Administrative/Operational & Clinical:
     a. Summary the need for and outcomes for special called RAC meetings:
                   HOTRAC has adjusted its meetings schedule to help reduce special-
                   called meetings. During the year of this report, HOTRAC has only
                   held one special-called conference call of the Board of Directors.
     b. Projected realignments of counties in trauma service area?
                   HOTRAC does not anticipate any realignments; however, HOTRAC
                   would support having the RAC and COG boundaries match. For
                   HOTRAC this would allow a better coordination for
                   emergency/disaster situations.
     c. Describe the RAC’s function prior to or during trauma center designations/re-
         designations within trauma service area that occurred within past twelve
         months?

                       HOTRAC supports its trauma facilities as needed. Since most of our
                       facilities have been trauma facilities since the State Trauma System
                       inception, typically HOTRAC only provides a support letter
                       outlining the facility’s participation in HOTRAC. However, we did
                       have a new Trauma Coordinator go through verification during this
                       report year, and HOTRAC provided technical assistance on meeting
                       some of the designation requirements.
        d.   Describe how the RAC administratively and operationally contributed to and
             participate in Injury Prevention initiatives within past twelve months. (Provide
             a brief summary of no less than three quantifiable events)
                       HOTRAC has been lucky in the fact that all of our members
                       complete multiple injury prevention activities throughout the year.
                       HOTRAC continues to offer its services to support these activities.
                       Typically we are asked to “get the word out” for various events (i.e.,
                       car seat checks, Shattered Dreams, health fairs, bike helmet give-a-
                       ways, etc.). Additionally, we provide Trauma Roo for attendance at
                       any requested event. HOTRAC collects monthly reports on injury
                       prevention activities that occur within the Region.
        e.   Describe the most significant findings of the RAC's SQI/Performance
             Improvement Committee within past twelve months.
                       HOTRAC did not find any significant findings this past year.
                       However, we did discover the need to implement full system PI for
                       9-1-1 EMS providers. That program was implemented in May 2008
                       and is providing needed information for the Region. We also
                       discovered the lack of documentation by FROs on a large MVC
                       occurring in rural counties. We are currently working on that issue.
                       We are also working to development a system to ensure that family
                       members are taken to the same facility if appropriate or at least in the
                       same direction (north, south).
        f.   To what degree were physicians in the trauma service area involved in the
             resolution of adverse patient care findings identified by the RAC's
             SQI/Performance Improvement Committee.
                       For the HOTRAC Region, our Physicians Advisory Committee
                       serves as our PI Committee. We have representation of the
                       Trauma/ED Medical Directors at the facilities as well as the EMS
                       Medical Directors and Physician Champions for pediatric, stroke,
                       and cardiac. All PI is reviewed by this committee whether in
                       summary or specific case review.
        g.   Describe activities the RAC was involved in to assist or encourage EMS and
             FRO participated in the RAC within past fiscal year (i.e. teleconferencing,
             video/conference calls, etc.).
                       HOTRAC has implemented conference call availability for any
                       monthly meetings upon request. Additionally, we held an EMS
                       Summitt in June 2008 to discuss any outstanding items that were not
                       addressed in the 1st or 2nd quarter meetings. We also use a lot of
                       email to keep our members abreast of issues.

        h. Identify problems or areas of concern identified in past twelve month that
            adversely impact RAC operations.
                     HOTRAC has no knowledge of such items during this period of this
                     report.
4. Is the information identified on Texas Secretary of State/Comptroller of Public
   Accounts (http://ecpa.cpa.state.tx.us/coa/Index.html) web site current? If not, what
   actions have been taken to ensure Certification of Franchise Tax Account Status
   (Registered Agent/Office) is current with the Texas Secretary of State/Comptroller of
   Pubic Accounts? yes

5. Issues/concerns that occurred in past twelve months that required technical assistance
   from the Office of EMS/Trauma System Coordination Group.
                       Location of an up-to-date EMS Medical Protocol Affidavit.
                          OEMS/TS was able to assist and provide the document.
                       Lack of EMS runs being noted for the FY08 EMS/County
                          funding received in June 2008. This item is still an issue but
                          OEMS/TS did everything they could. The issue is with the
                          Trauma Registry. HOTRAC was not funded to 2 of our most
                          productive providers.
6. What method will the RAC utilize to ensure member organizations receive a copy of
   this Annual Report?
                    HOTRAC will provide copies of the report at the December 4th
                    General Assembly Meeting.

                               ANNUAL BYLAWS AFFIDAVIT
                                      Attachment B


In accordance with Texas Administrative Code, §Rule 157.123, and Performance Measures for
EMS Funding allotments, each system should establish its authority commensurate with its ability
to provide trauma care.

        The RAC shall document an annual review of its bylaws. (§ Rule 157.123: Essential
        Criteria Defined. A.12)



Heart of Texas Regional Advisory Council (HOTRAC) has completed an annual review and/or
revision of the RAC’s Bylaws with a documented date of and ratified by member organizations
on 11/29/2007.

A current copy of the RAC’s bylaws is available for review on the RAC’s web site.

**HOTRAC will be bringing bylaws revisions to the General Assembly at its December 4, 2008
meeting for approval. Once approved, the new bylaws will be placed on the website and
submitted to the OEMS/TS.

                 ANNUAL REGIONAL TRAUMA SYSTEM PLAN AFFIDAVIT
                                  Attachment C


In accordance with Texas Administrative Code, §Rule 157.123, and Performance Measures for
EMS Funding allotments, each system should establish its authority commensurate with its ability
to provide trauma care.

        The RAC shall document an annual review of regional EMS/trauma system plan. (§ Rule
        157.123: Essential Criteria Defined. A.12)


Heart of Texas Regional Advisory Council (HOTRAC) has completed an annual review and/or
revision of the RAC’s regional trauma system plan with a documented date of and ratified by
approval from member organizations on 11/29/2007.
