1. INTRODUCTION

     Prompt and effective emergency response reduces accidental losses and the consequences of
     natural and man-made disasters. During an emergency there is not enough time to decide who
     is in charge, what outside help is required and who needs to do what to control the emergency
     situation. These issues must be addressed prior to the emergency.
     An effective emergency plan assigns responsibilities to specific individuals, establishes
     evacuation procedures, provides for notification to external agencies, establishes means of
     communications and provides guidelines for critical specific emergency situations.

2. OBJECTIVE

     The following are the main objectives of the ADGAS Emergency Response Procedures
     Manual (ERPM)
         To define the Emergency Organisation and the individuals functional role within the
         organisation.
         To provide clear and concise instructions to all Staff in the event of an emergency.
         To consider some specific emergencies and outline individual guidelines for response.

3. SCOPE

     This ERPM primarily applies to all Divisions/ Departments in ADGAS Das Island personnel
     who have an assigned responsibility for handling an emergency within ADGAS area of
     operation.
     This ERPM gives a broad outline of individual’s responsibility during an emergency and
     procedures for dealing with some specific emergencies.
     This ERPM by no means covers all eventualities and SHOULD be considered only as a guide
     and general source of information.
     For emergency involving Crisis Management Team, refer to Crisis Management Plan.
     For emergency involving ADMA-OPCO refer to the Facility Response Plan & ADMA-OPCO-
     ERPM

4. DEFINITIONS

     The following terms have been used in this manual to define a condition or course of action:

               SHALL
               The word 'SHALL' expresses a mandatory requirement for a condition or course of
               action.

               SHOULD
               The word 'SHOULD' expresses a strongly recommended or preferred condition or
               course of action.

               MAY
               The word 'MAY' suggests a possible course of action or condition where other options
               are available.
               WARNING
               A warning is used where failure to follow an instruction could result in serious injury or
               major incident.

               CAUTION
               A caution is used where failure to follow an instruction could result in injury or
               creation of a serious hazard.
               NOTE
               A note is used to draw attention to any matter of concern which may not be obvious, or
               to a procedure which enables the job to be performed more safely or effectively.
               BLUE ALERT
               This is a minor emergency which can be contained and is unlikely to escalate. The
               ADGAS Group Alert will not be activated.
               YELLOW ALERT
               This is an emergency which involves considerable hazard to personnel and/or plant
               with the potential for escalation to a serious emergency situation.
               The Plant or New Tank Farm siren may be actuated.
               An ADGAS Group Alert is given in these circumstances and ADMA-OPCO is
               informed.
               For any emergency situation if the ADGAS Group Alert is activated, the alert status is
               automatically considered to be “Yellow Alert”
               RED ALERT
               This is a serious emergency situation requiring the possible evacuation of all but
               essential personnel from Das Island.
               The Island sirens shall be actuated in these circumstances and an ADGAS Group Alert
               given.
               In the case of a red alert status, ADMA-OPCO Incident Controller becomes overall in
               charge.
               MINOR LEAK
               A hydrocarbon leak which is within the resources of the Operations Shift
               Superintendent (SS) to bring under control without calling for outside assistance, and
               without danger of escalation.

               MAJOR LEAK
               A hydrocarbon leak which requires outside assistance to control and protect plant and
               personnel, or is potentially uncontrollable.

               WARDEN
               Wardens are nominated for specific areas of the ADGAS facilities to control and direct
               the assembly and evacuation of personnel to a safe Marshalling Point.
               Designated Warden for each area are shown in the relevant Location Emergency
               Instruction.

               EMERGENCY SERVICES LIAISON OPERATOR (ESLO)
               An Operator attending the Marshalling Point at which the Emergency Services are
               assembling to act as the link between the Emergency Services and the Central Control
               Building (CCB).

               RELATIVE SAFETY
               A position of relative safety is one in which, although exposed to potential hazard, the
               risk is not immediate in nature.

               INJURY
               Injury is defined as an accident at work in which personal injuries are sustained.

               MAJOR INJURY
               A major injury is one in which the medical assistance is required on an urgent basis at
               the accident location by members of the Clinic Staff.
               MINOR INJURY
               A minor injury is one in which treatment can be carried out by qualified medical
               personnel at the hospital. Assistance from clinic staff is not required at the accident
               location. This is classified as a first aid case.
               RADIOLOGICAL PROTECTION SUPERVISOR (RPS)
               A person appointed by the PM to exercise special supervision with regards to the
               control and use of sources of ionising radiation.

5. POLICY STATEMENT
     ADGAS HSEMS requires that Crisis Management Plans SHALL be in place and that all
     potential emergency situations are identified assessed and have response plans which are
     exercised regularly.

6. GUIDE TO THE ADGAS ERP MANUAL
       6.1     RESPONSIBILITY FOR THE MANUAL
               The Health, Safety, Environment Department (HSED) is the custodian of this manual
               and is responsible for its maintenance and periodic upgrading. HSED will work closely
               with divisional and departmental managers and designated manual holders and ICS
               coordinator to ensure the following:
                       The establishment and promotion of procedures for maintaining the manual.

                       The co-ordination of revisions.
                       The distribution of revisions to manual holders.
               Manual holders are responsible for:
                       Keeping their copies updated by ensuring that all revisions are appropriately
                       inserted.
                       Studying all new material issued and incorporating it where relevant into their
                       work practices.
                       Suggesting changes and contributing new text material to improve the quality of
                       the manual.
                       Disseminating information contained in revisions.
                       Training personnel within their work area on applicable contents of the manual.
               •       ICS Coordinator for updating the FRP to ensure the latest updated copy of
                       ADGAS ERP Manual is enclosed.

       6.2     TRAINING
               Formal training on the ERPM will be conducted as part of the primary safety induction
               course for all new employees and employees being relocated to Das Island. For ICS
               training refer to binder “A” FRP

       6.3     DRILLS

               6.3.1      Introduction
                          In order to test ADGAS employees understanding and adherence to the
                          ERPM, and also to prove the actual procedures themselves with a practical
                          demonstration of their efficiency or need for reappraisal, three types of
                          Emergency Drills are performed on a regular basis on the LNG Plant.
                          These are the Mini Exercise, the Major Exercise, and the Crew Award
                          Exercise.
               6.3.2      Mini- Exercise
                          The Mini Exercise is designed to involve only Shift Operations Personnel, is
                          planned on a regular basis, and numbers around 20 exercises per year to give
                          all senior and junior crews equal participation. The Mini Exercise focuses on
                          a small scale emergency which is within the capability of the duty shift crew
                          to control, and does not require external assistance.




               6.3.3      Major Exercise
                          The Major Exercise is designed to involve Shift Operations Personnel, all
                          ADGAS Group Alert Personnel, plus Das Island Fire Service (DIFS). It is

                        planned on an irregular basis in order to inject the element of surprise, and
                        numbers at least three exercises per year, selecting a different senior crew and
                        a different scenario for each exercise.
               6.3.4    Crew Award
                        The Crew Award Exercise is held once per year for each senior and junior
                        shift operations crew. It is designed to test individual and group knowledge of
                        Emergency Plans and Procedures, and practical first aid response to
                        emergency situations with regard to fire fighting, gas containment, Breathing
                        Apparatus (BA), rescue and resuscitation. The Crew Award Exercise is
                        intended to stimulate operator interest and participation in the on-going
                        practical training from HSED and DIFS.
               6.3.5    Follow Up
                        Both mini and major exercises have a follow-up debriefing session, where
                        positive (and negative) actions are discussed, and positive recommendations to

       6.4     CONTRACTORS
               Contractors involved in work activities on Das Island SHALL also be given a briefing
               on the contents of the ERPM and SHALL, prior to starting work, be able to
               demonstrate competency in understanding the relevant aspects of the contents and its
               correct application.

       6.5     REVISIONS
               6.5.1 Revision request by ERPM holders
                     ERPM holders MAY initiate changes or add new text material by:
                              Completing a Revision Request form
                              Attaching the suggested text, graphics or photographs
                              Sending the form and new text to MHSE

               6.5.2    Review of Revision Request
                        HSED SHALL acknowledge, review and consider all new material for
                        inclusion in the ERPM in consultation with ICS Coordinator. The originators
                        SHOULD be advised of the final outcome of the request. Approved material
                        SHALL be published as a revision or as a bulletin if the revision is urgent, to
                        be incorporated later into the ERPM.
               6.5.3    Types of Revisions
                        HSED SHALL make required changes to the ERPM by issuing to manual
                        holders and FRP:
                              Normal revisions (on a periodical basis) and,
                              Bulletins (urgent revisions)

7. ORGANISATION

       7.1     COMMUNICATIONS
               7.1.1    Radio Communication
                        In the event of an emergency being declared,
                        ALL PERSONNEL SHALL SWITCH TO CHANNEL ONE.
                        Radio communication SHOULD                 be   restricted     to      ESSENTIAL
                        INFORMATION ONLY.
                        The Head of Shift Operations SHALL have priority on all radio
                        communications.
                        Fire & Rescue Team Das (FRT) SHOULD normally use their own radio
                        channel but MAY also have the facility to use the ADGAS channel.
                        An ADGAS radio, which has been issued to the FRT, SHALL be available at
                        the Forward Control for authorised users.
                        The fixed radio in the ICC are to be used for listen mode only unless under
                        adverse circumstances as directed by I incident Commander.

               7.1.7    Identification of Personnel

               7.1.8    Communication of Incidents to Local Authorities
                        The requirement for reporting of any incident to local authorities is covered in
                        detail in ADGAS HSE Manual Chapter 10 and this is generally within 24
                        hours to the various authorities. However, there is an exception regarding
                        fires of any magnitude e.g. full fire, a single flame, smouldering, or burning,
                        these must be reported immediately to the Critical National Infrastructure
                        Authority (CNIA). The mechanism for this reporting is through Head of Shift
                        Operations (HSO) to duty OSE who will inform the Personnel Officer
                        (Relations) who in turn will inform the CNIA of the event.
                        In the event of any fire, including late fire calls the Head of Shift Operations
                        (HSO) must call the duty OSE who will initiate the necessary reporting
                        through PO(R).


               7.1.9    Accident & Incident Investigation
                        All accidents & incidents including fires, injuries, and environmental damage or
                        asset damage will be investigated by ADGAS through the Accident & Incident
                        reporting procedure to allow lessons to be learned and improvements to be
                        made. Only cases of suspected criminal damage will be handed to the police for
                        investigation. The Police and State Security will be kept informed of the
                        outcome of any ADGAS investigation into Reportable Accidents (see Chapter
                        10, Section 8).

       7.2     EMERGENCY ORGANISATION

               7.2.1(a) Organisation
                        The emergency organisation is shown on page no. 21 in Figure 1, and is
                        followed by a statement detailing the individual position's Location, Function,
                        Reporting Role, and Initial Response to plant emergencies.

                        In the event of loss of ICC, ADGAS Emergency Team will use Oasis Club.

               7.2.1(b) Das Emergency Plan Manning Requirements

                        In order to have an effective operational emergency plan, emergency positions
                        are required to be filled at various locations on Das as defined in Fig. 1
                        Emergency Organisation. The Table 1a below does not cover every
                        emergency position, relating to ADGAS on Das, however it does provide the
                        arrangements for coverage of key day positions that are seen as essential for
                        operation of the emergency organisation. On the other hand Table 1b is used
                        by both HSED and ICS Coordinator on a weekly basis showing the availability
                        of each Emergency Response Team Member for the corresponding period.

                               Incidents that are multi-jurisdictional, or involve several agencies, may
                               require the establishment of the Liaison Officer position on the
                               Command Staff. The Liaison Officer is the point of contact for the
                               assisting & cooperating Agency Representatives & stakeholder groups.
                               Only one Liaison Officer will be assigned for each incident, including
                               incidents operating under Unified Command & multi-jurisdiction
                               incidents. The Liaison Officer may have assistants as necessary, & the

                               During emergencies (Act of Terrorism and threats not included) on Das
                               Island, the LIO will be the link between the local authorities, (including
                               Das Police), and the Das Island IMT.

                               link between the local authorities and the IMT.
                               The LIO on Das Island will normally be the Senior Government
                               Relations Officer

                   XI.    INFORMATION OFFICER
                          (ICS COORDINATOR)


                          Location:

                          Incident Commander Centre (ICC)


                          Functions:

                          The Information Officer is responsible for developing & releasing
                          information about the incident to the news media, to incident personnel, &
                          to other appropriate agencies & organizations. Only one Information
                          Officer will be assigned for each incident, including incidents operating
                          under Unified Command & multi-jurisdictional incidents. The IO may
                          work with IOs from other directly involved response organizations to form
                          a Joint Information Center (JIC) when Unified Command has been
                          activated. When this occurs, one of the IOs becomes the Incident
                          Information Officer (IIO), & the others become assistants. The
                          Information Officer may have assistants as necessary, & the assistants
                          may also represent assisting agencies or jurisdictions. On Das Island, the
                          IO will release information (approved by the IC) to the Crisis
                          Management Team Only.

                            Function:
                            Directs the resources of the DIFS to achieve the objectives determined by
                            the HSO. Responsible for fire fighting, gas control and search/rescue
                            activities.
                            Is responsible for initiating the call out to Emergency Teams of ADGAS
                            and ADMA-OPCO whenever an emergency escalates to yellow alert.
                            To consult with HSO and Incident Commander before declaring “RED
                            ALERT” status.


                            Reports to:
                            ESLO on arrival and subsequently to HSO.



                            Initial Response:
                                 Proceeds to designated Marshalling Point where he assumes
                                 responsibility for all fire fighting and search/rescue activities.

8. LOCATION EMERGENCY INSTRUCTIONS

      8.1      SUPERVISOR'S RESPONSIBILITIES
               It is the responsibility of all supervisors to account for the number and location of
               personnel working under their control on the ADGAS facilities.
               In the event of an emergency the immediate supervisor SHALL ensure that all
               personnel under his control are safely evacuated to the appropriate Marshalling point.

      8.2      EVACUATION OF LNG PLANT
               On the sounding of the Plant General Alarm, personnel, other than those directly
               involved in the emergency, e.g. from Production Division, SHALL take the following
               actions:
                  Stop all work.
                  Shutdown all mobile maintenance equipment.
                  Advise and give assistance to remove personnel working in confined spaces.
                  Proceed windward otherwise - across when in the gas cloud to the nearest safe
                  Marshalling Point using an escape respirator.
                  Await further instructions.

      8.3      CENTRAL CONTROL BUILDING (CCB)

               8.3.1    H2S Alarm
                        On the sounding of the H2S Alarm in the CCB personnel SHALL take the
                        following actions:
                              Stay at their place of work within the Control Room. The fresh air intake
                              of HVAC system will close upon detection of H2S.
                              Await instruction from Duty Shift Superintendent.

               8.3.2    Plant General Alarm
                         On the sounding of the local alarm or Plant General Alarm non-essential
                         personnel SHALL take the following actions:
                              Leave their place of work.
                              Shut, but do not lock, doors.
                              Proceed to nearest safe Marshalling Point as directed by WARDENS
                              using escape respirator if required.
                              Await further instructions.

      8.4      EVACUATION OF QUARRY MAINTENANCE BASE (QMB)

               8.4.1    Personnel
                        On the sounding of the Quarry Maintenance Base Klaxon or the Plant General
                        Alarm, personnel SHALL take the following actions:
                              Leave their place of work or office.
                              Shut, but do not lock, doors.
                              Assemble in the general working area with escape respirator and await
                              instructions from the WARDENS.
                              Proceed to the nearest safe Marshalling Point as directed by
                              WARDENS, using escape respirator.
                              Await further instructions.

               8.4.2    Supervisor
                              First available Supervisor SHALL assume the responsibilities of
                              WARDEN and SHALL carry out the following actions:
                                    Don fluorescent waistcoat *
                                    Pick up loud hailer *
                                    Pick up radio *
                                    Direct other available Supervisors to assist
                                    Contact CCB for guidance
                                    * Available on mezzanine floor walkway.
                              All other available Supervisors SHALL assist the WARDEN to ensure
                              the following actions are carried out:
                                    Building is totally evacuated
                                    Machinery is isolated
                                    Crowd is controlled
                                    Any other assistance as requested by the WARDENS
      8.5      EVACUATION OF STOREX
                       Stop all work.
                       Shutdown all mobile maintenance equipment.
                       Persons on tank tops SHALL return to ground via the stair tower and SHALL
                       not use the lifts.
                       Proceed across wind, using escape respirator if required, to the nearest safe
                       Marshalling Point.
                       Await further instructions.

      8.6      STOREX CONTROL ROOM

               8.6.1 H2S Alarm
                     On the sounding of the Hydrogen Sulphide Alarm personnel SHALL take the
                     following actions:
                            Stay at their place of work within SCR. (The fresh air intake of the
                            HVAC system will close upon detection of H2S.)
                            Await instructions from HSO/HOS.

               8.6.2 Evacuation
                       On the sounding of the Local Fire or CO2 pre-release alarm, personnel SHALL
                       take the following actions:
                            Leave their place of work.
                            Shut, but do not lock, doors.
                            Proceed to the nearest safe Marshalling Point using escape respirator if
                            required.
                            Await further instructions.

      8.7      EVACUATION OF LNG JETTY
               On the instruction of the Jetty Operator, non-essential personnel SHALL take the
               following actions:
                       Stop all work.
                       Shutdown all mobile maintenance equipment.
                       Proceed to Marshalling Point 5 using escape respirator if required.
                       Await further instructions.

      8.8      EVACUATION OF SULPHUR JETTY
               On the instruction of the Jetty Operator, non-essential personnel SHALL take the
               following actions:
                       Stop all work.
                       Shut down all mobile maintenance equipment.
                       Proceed to Marshalling Point 8 at Sulphur Storage Facility using escape
                       respirator if required.
                       Await further instructions.

                       Evacuate the building.
                       Assemble in area Car Park between Projects and OMA Annexe buildings.

      8.11 EVACUATION OF AL JIMI ACCOMMODATION
               On the sounding of the local Fire Alarm personnel/ resident SHALL take the following
               actions:
                       Leave their place of work or room.

                       Evacuate your area to the nearest Marshalling Point.
                       Await further instructions.

      8.13 EVACUATION OF ADGAS MAIN OFFICE BUILDING (DAS)
               On the sounding of the local Fire Alarm, personnel SHALL take the following actions:
                       Leave their place of work or office.
                       Shut, but do not lock, doors.
                       If safe to do so, attack fire with available fire extinguisher.
                       Call Fire Service, 63333 from safe area.
                       Evacuate the building through the nearest fire exit.
                       Assemble at either Marshalling Points (North or South side)
                       Await further instructions.
               Designated WARDENS: As posted in each floor

      8.14 EVACUATION OF TRAINING CENTRE
               On the sounding of the local Fire Alarm, personnel SHALL take the following actions:
                      Leave their place of work or office.
                      Shut, but do not lock, doors.
                      If safe to do so, attack fire with available fire extinguisher.

                       Evacuate the building.

                       If safe to do so, attack fire with available fire extinguisher.
                       Telephone Fire Service, 63333 from safe area.
                       Evacuate the area by the shortest route to open air.
                       Assemble in the open area North side away from CWS.
                       Await further instructions.
               Designated WARDENS: As posted in the building

      8.17 EVACUATION OF MATERIALS STORE
               On the sounding of the local Fire Alarm, personnel SHALL take the following actions:
                      Ensure equipment/machines are switched off.
                      Leave their place of work or office.
                      Shut but do not lock doors.
                       If safe to do so, attack fire with available fire extinguisher.
                       Telephone Fire Service, 63333 from safe area.
                       Evacuate Stores Area.
                       Assemble on flat area between CWS and Craft Training Centre.
                       Await further instructions.

9. SPECIFIC EMERGENCY ACTIONS

      9.1      MAJOR LEAK OF HC/ TOXIC GAS – LNG PLANT

               9.1.1    Operators (Plant)
                        Upon discovering the hazard moves across wind to a position of RELATIVE
                        SAFETY.
                        Informs CCB by radio, awaits acknowledgement.               If not received, calls
                        Telephone 62222 stating:-
                              "Emergency Gas Leak, Emergency Gas Leak".
                              Location of Leak.
                              Details of any known casualties.
                        Dons BA.
                        Stops all hot work and engines.
                        Assists in the accounting of personnel.
                        Rescues personnel if location is known.
                        In liaison with CCB, commences appropriate isolations and deployment of gas
                        cloud control equipment.
                        Assists in the search and rescue of missing personnel.

               9.1.2    Train Shift Supervisor
                        Acknowledges and repeats message.
                        Informs HSO of wind direction and Marshalling Points that can be used.
                        HSO SHALL decide the Marshalling Point to be used.
                        Telephones ESCR on direct line stating:
                              "Gas Leak, LNG Plant, Gas Leak, LNG Plant.
                              Report to Marshalling Point No. ......".
                        Sounds Plant General Alarm if directed by HSO.
                        Gives general call over radio to all Operators stating:-
                              "Gas Leak at . . . . Wear BA, Gas Leak at . . . . Wear BA".
                        Liaises with HSO to ascertain measures to be taken.

               9.1.3    Power Shift Supervisor
                        Informs: OC (U).
                        On instruction from HSO activates Group Alert System and/ or Plant siren.
                        Contacts OSE stating:

                        Stops loading ship and unberths if so directed by Shift Supervisor(s)

               9.1.5    Head of Shift Operations
                        Ensures immediate notifications have been made.
                        Dispatches an ESLO to the designated Marshalling Point to direct the Fire
                        Service.
                        Liaises with Operator(s) at site to determine extent and nature of the leak.
                        Decides on Plant Siren activation.
                        In consultation with the FRTL declares alert status and location of Forward
                        Control.
                        Considers shutdown requirements of LNG Plant, fired heaters and pilot
                        burners.
                        Directs Shift Supervisor(s) to stop loading and, if appropriate, unberth ship.
                        Considers the shutdown of boilers taking due account of wind direction. If
                        necessary, shutdown Utilities completely.
                        Considers the shutdown of electrical generation except that to power essential
                        services.
                        Directs Umm-Shaif Supervisor to isolate LNG gas supplies if necessary.
                        Establishes the approach to control the emergency with FRTL and directs the
                        OSE to deploy his men as appropriate.

                        NOTE:
                        In the event of a MINOR LEAK of HC or toxic gas the HSO, at his discretion,
                        MAY inform the FRTL of the situation and confirm that no attendance by the
                        Fire Service is required".
                        If the FRTL is contacted, the OSE SHALL also be contacted.



               9.1.6    Operations Safety Engineer

                        Liaises with HSO providing assistance as required.

               9.1.9    Fire & Rescue Team Leader
                        Proceeds to designated Marshalling Point where he assumes responsibility for
                        all actions relevant to control the emergency including search and rescue.
                        Establishes Forward Control in consultation with the HSO.
                        In consultation with the HSO, establishes the approach to be taken in
                        controlling the emergency.
                        Acts within his discretion in all activities to control the emergency and carry
                        out search/rescue.
                        Inform the ESCR of Alert Status.



      9.2      OUTBREAK OF FIRE – LNG PLANT

               9.2.1    Operators (Plant)
                        Moves to an area of RELATIVE SAFETY.
                        Informs CCB by radio, awaits acknowledgement.              If not received calls
                        telephone 62222 stating:
                              "Emergency – Fire, Emergency - Fire".
                              Location of fire.
                              Details of any known casualties.
                        Assists in the accounting of personnel.
                        Stops all hot work and engines.
                        Rescues personnel if location is known.

               9.2.3    Utilities Shift Supervisor (CCB) - Power Desk
                        Informs Operations Controller (Utilities).
                        On instruction of the HSO activates Group Alert System and gives a voice
                        message clearly stating:
                             Location of fire.
                             Report to Marshalling Point No. ....
                        Contacts ADMA-OPCO ESCR by telephone and states:
                             "Fire at LNG Plant, Fire at LNG Plant,
                             Report to Marshalling Point No. ....."
                        Ensures that fire pumps have been started and cut in satisfactory and fireman
                        pressure is adequate.

               9.2.4    Head of Shift Operations
                        Ensures immediate notifications have been made.
                        Dispatches an ESLO to the nominated Marshalling Point to direct the Fire
                        Service.
                        Liaises with Operator(s) at site to determine extent and nature of fire.
                        Decides on Plant Siren activation.
                        In consultation with FRTL (Fire & Rescue Team Leader) declares Alert
                        Status and location of Forward Control.
                        Considers LNG Plant shutdown requirements.
                        Establishes the approach to firefighting and rescue operations with FRTL and
                        directs the OSE to deploy his men, as appropriate.

                        Late Fire Call:
                        Where a minor fire or smouldering has been extinguished without the
                        attendance of the Fire & Rescue Team Das a "Late Fire Call" SHALL be
                        made. The contact SHOULD be via the direct line and the following message
                        sent:

                        Minor Fires (i.e. Fires which are immediately extinguished)
                        Head of Shift Operations will instruct Utilities Shift Supervisor (CCB) to
                        inform ADMA CCR.

                        ADGAS duty HSE must be informed of any late fire call so that he can inform
                        ADGAS Personnel Officer (Relations) who will inform Das Police of the
                        event

               9.2.5    Operations Safety Engineer
                        Proceeds to designated Marshalling Point.
                        Awaits instruction from HSO and, if requested, assists in firefighting and
                        rescue activities as directed by the FRTL (Fire & Rescue Team Leader).

               9.2.6    Operations Controller (Process/ Utilities)
                        Proceeds to site and directs the operations in liaison with HSO.
                        Implements HSO instructions relating to plant shutdown and firefighting
                        activities.

               9.2.7    Head of Process /Head of Utilities
                        Proceeds to marshalling point and if safe to CCB or as directed by HSO.
                        Ensures that all emergency notifications have been completed.
                        Liaises with Forward Control providing assistance as required.

               9.2.8    Fire & Rescue Team Leader
                        Proceeds to designated Marshalling Point where he assumes the responsibility
                        for all firefighting and search/rescue activities.
                        Establishes Forward Control in consultation with the HSO.
                        In consultation with the HSO, establishes the approach to be taken in
                        controlling the emergency.
                        Acts within his discretion in all firefighting and search/rescue activities.
                        Informs the ESCR of Alert Status.




      9.3      MAJOR LEAK OF HC – STOREX

                        Sounds the Storex general alarm on instruction of the SS.


               9.3.5    Process Shift Supervisor (CCB)
                        Acknowledges and repeats message.
                        Gives general call over radio to all Operators stating:
                              "Gas Leak at Wear BA, Gas Leak at Wear BA"
                        Informs HSO of the HC leak and wind direction.
                        Sounds the LNG Plant Alarm Sirens, if so instructed by HSO.
                        Informs Umm Shaif Control Room
                        Liaises with HSO to ascertain measures to be taken.

                         controlling the emergency.
                         Acts within his discretion in all activities to control the emergency and carry
                         out search/ rescue.
                         Inform the ESCR of Alert Status.

                        Enter the water only as a last resort or on specific instructions from HSO or
                        SS(S) only if capable of swimming and using safety harness/ lifeline and life
                        jacket.

               9.7.2    Storex Control Room Operator
                        On receiving message from the Jetty Operator contacts ADMA-OPCO Marine
                        on VHF radio channel 16 and then transfers to channel P2. Communicates the
                        following message.
                              "Emergency, Emergency Man in water at ........... Request assistance".
                        Informs SSS.

               9.7.3    Storex Shift Supervisor
                        Liaises with HSO.
                        Proceeds to the incident scene.
                        Maintains contact with Das Marine on channel P2 using portable VHF set
                        (located in Operator's cabin at Jetty).

      9.11 LOSS OF RADIOACTIVE SOURCE

               9.11.1    Introduction
                         Radioactive sources are used primarily by two Sections within ADGAS, the
                         Instrument Section of Maintenance Dept and C&ID. In the event of any
                         suspected loss of, or damage to, any sealed source of radioactive material, the
                         following procedure shall be followed or as advised by the RPS.

               9.11.2    Instrument Engineer/ Plant Inspection Engineer
                         Informs CCB by telephone 62222.
                         Ensures that all personnel leave the immediate vicinity of the source.
                         Erect barriers carrying warning notices defining the hazardous area.
                         Assist the RPS in reduction of radiation levels and retrieval of source.

               9.11.3    Process Shift Supervisor (CCB)
                         Informs HSO.
                         Informs RPS stating location of emergency and the reporting point.

               9.11.4    Power Shift Supervisor
                         Contacts OSE stating nature and location of emergency.

               9.11.5    Head of Shift Operations
                         Ensures immediate notification have been made by Shift Supervisor Utilities
                         SS(U) and Shift Supervisor (P).
                         Ensures all personnel have evacuated the area by liaising with OC(P) or
                         OC(U).

                         Alert the Fire Services immediately by emergency telephone No.63333 or
                         radio to the CCB and activate the break glass alarm (where applicable).
                         Say,       "Emergency - Fire at Power Station",
                                    "Emergency - Fire at Power Station".
                         Give precise details stating:-
                                Location of fire, e.g. inside turbine canopy, turbine hall, secondary
                                scrubbers, main gas skid, switch rooms, offices, control rooms, etc.
                                Type of fire, e.g. pump gland, flange leak, electric, etc.
                                Details of any known injuries.
                                In case of small fire attempt to extinguish using the available fire
                                extinguisher.
                         In the event of a major fire:-
                                Restrict access of personnel and vehicles.
                                Informs HSO/HOU/OC(U) by radio.

                         CAUTION:
                         Under no circumstances SHALL a person take undue risks in attempting to
                         extinguish a fire.
                         The following specific actions SHALL be taken by Operations staff for fire in
                         the areas listed below:

                         i      Fire Inside Turbine Canopy
                                GT shuts down and release CO2 automatically, if not use manual
                                procedure.
                                Shut off gas and fuel oil supplies to GT.
                                Vent gas line to atmosphere and drain fuel oil line.
                                Leave all doors closed and standby for Emergency Services.

                               If the fire makes isolation impossible contacts ADMA production to
                               isolate and vent at the Production Plant.

                         v     Fire at Switch Rooms
                               Evacuate all personnel from area concerned.
                               Auto fire protection SHALL operate. If not then operate on manual.
                               Leave all doors closed and standby for Emergency Services.

                         vi    Fire at Office and Control Room
                               Evacuate all personnel from area concerned.
                               Leave all doors closed and standby for Emergency Services.

               9.13.2    Gas or Oil Leak
                         Don BA.
                         Move across wind to a safe location.
                         At the earliest opportunity alert persons in the immediate vicinity and raise the
                         alarm.

      9.14 SEA WATER LAYDOWN AREA (TEMPORARY WASTE STORAGE SITE)

               Where a minor fire smouldering has been extinguished without the attendance area of
               Das Island Fire Service, a late fire call should nevertheless be made. The contact
               should be via the direct line.

      9.15. MAJOR LEAK OF LIQUID SULPHUR – LNG PLANT – PLANT 7

               9.15.1    Operators (Plant)

                         Move to an area of relative safety.

                         Inform CCB by radio, await acknowledgement. If not received, phone Tel.
                         62222 stating:
                         “Emergency – Sulphur leak, Emergency – Sulphur leak.”

                         Location of the sulphur leak and extent of leak.

                         Fire & Rescue Services is not required a “Late Fire Call” shall be made. The
                         contact should be via the direct line and the following message sent.

                         “This is not an emergency. I repeat this is not an emergency. A minor
                         sulphur leak occurred on the LNG Plant. The leak has been stopped.
                         Please report to Marshalling Point _____________

               9.15.5    Operations Safety Engineer

                         Proceeds to designated Marshalling Point.

                         Awaits instruction from HSO and, if requested, assists in fire fighting,
                         containment and rescue activities as directed by FRTL

               9.15.6    Operations Controller (Process)

                         Proceeds to incident location and directs the Operations in liaison with HSO.
                         Assumes responsibilities of Incident Controller.

                         Implements HSO instructions relating to Plant shutdown, fire fighting and
                         containment activities.

                         activities.

      9.16 MAJOR LEAK OF LIQUID SULPHUR – RUNDOWN LINE

               9.16.1    Operators

                         Move to an area of relative safety.

               9.16.2    Storex Shift Supervisor

                         Contacts CCB and liaises with HSO to minimise the leak.

                         Proceeds to the area of the sulphur leak in the Storex shift vehicle and closes
                         off the road if necessary.

                         Informs Sulphur Ship Master if a ship is alongside Berth No. 5.

               9.16.3    Head of Storex

                         Proceeds to nominated Marshalling Point then onto the ADGAS Storex
                         Control Building by the safest route.

                         Liaises with HSO and OC(S) in the field to contain liquid sulphur spillage and
                         shutdown/isolate plant as required.

                         Ensures correct Safety Equipment / PPE is utilised when evaluating the leak.

               9.16.4    Train Shift Supervisor

                         Acknowledges and repeats message.

                         Informs HSO.

                         sulphur leak on the rundown line to storage.

                         Gives exact location of the leak.

                         Liaises with the Shift Supervisor(S) and Operator(s) at the leak location to
                         determine severity of the leak and hazards it is creating.

                         In consultation with FRTL declares Alert Status and location of Forward
                         Control ( following arrival of HO in CCB ).

                         Considers LNG Plant / Sulphur Plant shutdown requirements.

                         Establishes the approach to containment, fire fighting and rescue operation
                         with FRTL and directs SOSE to deploy his men, as appropriate.

                         Late Fire Call

                         Where a minor liquid sulphur leak from the rundown line has stopped without
                         the attendance of F&RS(D) a “Late Call” shall be made. The contact should
                         be via the direct line and the following message sent.

                         If not acknowledged, telephones 62222 repeating the above message.

                         If a ship is on the berth, informs the Berthing Master and Ship’s Master (by
                         Marine Radio which is on the International channel).

                         Prepares to disconnect loading arms.

                         Unberths ship if instructed by HSO.

                         Attempts to isolate the leaking line.

               9.17.2    Senior Operator (Storex)
                         Remains in Storex Control Room (SCR) monitoring shutdown of equipment.
                         Takes action as requested by Shift Supervisor(Storex).

               9.17.3    Shift Supervisor(S)
                         Ensures appropriate measures are taken to contain the leaking molten sulphur.
                         Sounds Storex General Alarm on instruction of HSO.

                         In conjunction with FRTL declares Alert Status.

                         Liaises with FRTL at the incident scene following handover to HOP in CCB.

                         Establishes the approach to control/contain the leak with the FRTL and directs
                         OSE to deploy his men as appropriate to assist in the control and containment
                         of the Liquid Sulphur Leak (at the Sulphur Jetty or in the Sulphur Storage
                         Area).

                         Late ‘Fire” call.

                         Where a liquid sulphur leak has stopped or is considered minor and the Das
                         F&RS are not required to attend a “Late Fire Call” shall be made.

                         Takes control in CCB when HSO proceeds to incident scene.

               9.17.9    Head of Storex

                         Liaises with HSO providing assistance, as required, at the incident scene.

                         Master for any actions required.

               9.17.10 Fire & Rescue Team Leader

                         Establishes Forward Control in consultation with HSO (Sulphur Jetty or
                         Sulphur Storage Area).

                         In consultation with HSO establishes the approach to be taken to isolate and
                         contain the liquid sulphur spillage.

                         Acts within his discretion in all liquid sulphur containment activities.
