Pursuant to Section 25 of the 2008-2009 Department of Juvenile Justice and Delinquency (DJJDP) Agreement for Support our Students funds, the Grantee shall not subcontract
any of the work or services provided in the Agreement without the expressed prior written approval of the Grantor for the expenditure of funds for subcontract services greater than
$1000.00.

The Department provides approval for the expenditure of funds for the purchase of subcontracted services and other contracted services only. In approving this document, the
Department is NOT approving the subcontract nor its terms and conditions. The Grantee is required to maintain on file a copy of all subcontract agreements regardless of cost and
shall make subcontract agreements available for Department review upon request.

Failure to obtain prior approval for expenditure of funds for subcontract services greater than $1,000.00 may result in a reversion of funds for the subcontracted services.

Directions:

1. List all subcontracted services on this form regardless of cost. Please do not enter Contracted Personnel information on this form.

2. Provide vendor name, service description, length of contract and cost for each subcontract service. Please submit an itemized list of subcontracted services.

3. Ensure that the purpose and description of the subcontract service is thorough to include its overall connection to the Program Plan.

4. Indicate N/A if there are no proposed subcontract services.

5. Add rows as needed.

6. Retain a copy of all pages of this form prior to submitting to DJJDP for approval.

7. Submit all pages of this document to DJJDP.
8. Should Grantees wish to enter into subcontract/subgrant agreements over $1,000 throughout the DJJDP contract period, this form is required to be submitted for each request to
approve expenditure of funds for subcontracted/subgranted services over $1,000 and/or to approve any modifications of subcontracted services already approved.

9. Form must be submitted with Budget Revisions if there are proposed changes to contracted services.
