 I am a new Helios customer and I understand that I am entitled to FREE software and downloadable updates and upgrades when they
 are released as long as I have an active Software Maintenance Agreement for all of my locations. I also understand that I will receive
 FREE telephone support from 8am-8pm Central time Monday-Friday (excluding holidays) and 10am-2pm Central time Saturday by
 calling (317) 554-9911.


 We have cancelled a maintenance agreement on a previous version and understand we are subject to a $399 enrollment fee and all
 applicable back months per location. I am entitled to FREE downloadable software updates and upgrades when they are released as
 long as I have an active Software Maintenance Agreement for all of my locations. I also understand that I will receive FREE telephone
 support from 8am-8pm Central time Monday-Friday (excluding holidays) and 10am-2pm Central time Saturday by calling (317) 554-
 9911.

   1.  All communication regarding product announcements and company policy changes will be done via email.
   2.  All prices, fees and draft rates are NOT refundable and are subject to change without notice.
   3.  All fees are paid in advance and due on the 1st of each term (month or year) and will be debited without notice from HELIOS, LLC ™.
   4.  HELIOS, LLC™ reserves the right to cancel or modify my membership at any time.
   5.  HELIOS, LLC ™ reserves the right to assign this authorization without notice.
   6.  HELIOS, LLC ™ reserves the right to change the membership policies and privileges at any time without notice.
   7.  The total liability of HELIOS, LLC on any and all claims arising under this agreement shall not exceed the cost of one (1) month's service.
   8.  All cancellation letters must be received at least 30 days before the next scheduled debit date otherwise one final debit will be made as scheduled.
   9.  This agreement is for one location only and I understand that I must sign up ALL of my locations or this contract will either not be accepted or cancelled.
   10. FREE Technical Support is provided from 8am-8pm Central time Monday-Friday (excluding holidays) and 10am-2pm Central time on Saturday by calling
       (317) 554-9911. After hours emergency support is a flat fee per incident and will automatically be deducted from your account.
   11. FREE Technical Support is provided on Helios (WAN & LAN) and Bookyourself.com under this agreement. Front line support related to computer
       hardware, Windows operating system, local-area-networking and broadband is also included in this agreement.
   12. Helios version 11 requires that you have a broadband internet connection (DSL, cable, satellite) for appointment book, credit card processing, WAN, E-
       commerce, web reporting, back office access, remote back-up and will only be supported on Windows 2000 Professional and Windows XP Professional.
   13. All delinquent and cancelled accounts will result in service interruption of Bookyourself.com.

  This authority is to remain in effect until HELIOS, LLC ™ receives a written and signed cancellation notice on company letterhead via certified mail or
