                                    INTERAGENCY REFERRAL FORM (HS 10)

Purpose: To document and communicate any referrals made to any community
agency.

Procedure: This form is to be used by Regional personnel if there are any
atypical results in the Child Health Record especially Form 6, Part II or Form 3
(atypical results such as hearing/vision referral, repeat Hematocrit or
Hemoglobin, repeat lead levels, abnormal height or weight, physician
referrals/follow-ups, food allergies, referral to WIC, etc…). Send original to
referee and file the copy in the child’s folder.
