SAMPLE LETTER TO CURRENT VOLUNTEERS
RE-ENROLLING IN EXTENSION YOUTH PROGRAM
(after 5 years from last screening)
Another successful and exciting 4-H year has just come to an end. We have
experienced individual and group accomplishments. It is exciting to be a part of a quality
system in which youth and adults work together to help the young people develop skills
to be contributing and productive members of the community, country and world. Thank
you for helping make these opportunities possible for youth in our county.
The Illinois 4-H Youth Development program requires reviewing volunteer applications
every five years. The procedures provide greater protection for our members and
volunteers. At this stage we request you complete the enclosed copy of Department of
Children and Family Services Authorization of Background Check form and the
Conviction Information Name Check Request for the Illinois State Police. Be sure to sign
and date them. Return the two completed forms to the Extension office in the enclosed
envelope. University of Illinois Extension policies require that CANTS applications and
Conviction Checks be processed and on file before we re-enroll you as a leader.
We believe that the youth protection procedures make a public statement of our
dedication to the well being of youth and increase the probability of a continued positive
image for 4-H. We are sure that you would want to know that any salaried or volunteer
staff working with your own children or grandchildren has been carefully selected.
