ECPE June 2009 Announcement and Order Information 1) Submit the June 2009 ECPE Order Form by DECEMBER 1, 2008. Please send by mail, by fax (734.615.6586, using the paper form you have been sent) or via the ELI-UM Web site, www.lsa.umich.edu/eli/testing. Orders received after December 1, 2008 cannot be filled. 2) Contact Information: Please provide your current address, telephone, fax numbers, and e-mail address. If you do not plan to administer the ECPE in June 2009, provide your contact information and also write “Not testing in June 2009.” 3) Order bulletins and test booklets in multiples of 25, according to your needs. If excessive amounts are ordered, we will reduce quantities to a reasonable figure. Orders should be based on the number of exams administered in previous years. If you expect a great increase or decrease in the number of candidates, please include an explanation. 4) Please let ELI-UM know if the current mailing address we have for you is incorrect or incomplete in any way. ELI-UM sends test materials via private courier to all test centers. We must have your complete street address in order to use the courier service. 5) Test fees, payment procedures, and policies are published in the Examiner’s Manual.
