The Dental Hygiene curriculum is both exciting and challenging, it offers graduates the opportunities for
employment in a variety of settings. Your degree will provide you with a lifelong career that is rewarding
and personally fulfilling.

The requisites for success in the program include a love of science, good manual dexterity, and a strong

The program demands may pose unique personal as well as academic challenges. Most of our students
have multiple responsibilities in addition to going to school (i.e., married, family, job). Success in the
program takes a firm commitment, but the Dental Hygiene faculty is here to help you succeed.

We strongly suggest you prepare well by doing the following:
• Visit and talk to dental hygienists in various dental clinics
• Attend one of our program information meetings. Please see the Program Outline for scheduled dates
  and times
• Talk with an advisor/counselor in order to help you with your career plans
• Prepare an economic plan to assist you in meeting program costs

CAREER DESCRIPTION: Dental hygienists work under the supervision of a dentist. Their
responsibilities include: oral health assessments, head and neck examinations, oral hygiene instructions,
periodontal and preventive therapies, radiographic procedures, community service activities, local
anesthesia, placing restorations and other expanded functions. Dental hygienists must complete a
minimum of a 2-year education program, usually offered in a community or junior college. The course of
study includes general education courses, technical courses on dental hygiene procedures, and clinical
experience. Graduates receive an Associate of Applied Science degree. A 4-year Bachelor of Science
degree is also available at select universities.
PROGRAM OBJECTIVES:
• Prepare dental hygienists who will be able to effectively assess, plan, implement, and evaluate current
  dental services.
• Develop appropriate decision-making skills and utilize professional judgment, conduct, and ethics to
  provide optimum patient care.
• Encourage flexibility and innovative approaches to problem solving, which stimulate independence
  and develop personal and professional responsibility.
• Enhance communication and interactive skills, which enable students and faculty to work effectively
  with diverse populations as members of the health care team.
• Advocate active participation and leadership in community activities and professional associations.
• Instill a commitment to continued education and skill development.
LENGTH OF PROGRAM: two-years for a total of 6 terms (Currently, no Dental Hygiene classes are
offered summer term between the first and second year. This may change in the near future).

TRANSFER CONSIDERATIONS TO 4-YEAR INSTITUTIONS: Students interested in a Bachelor of
Science Degree may transfer to many local colleges and universities. A degree completion program is
currently available through Eastern Washington University at the Clark College Campus in Vancouver,
Washington, and Oregon Institute of Technology. Numerous online programs are also available.

ACCEPTED STUDENTS
IMMUNIZATIONS: Accepted students must have verification of measles (Rubella) and chicken pox
immunization, tuberculosis status and have begun a Hepatitis B immunization series prior to beginning
Fall Term, first year.
CRIMINAL BACKGROUND CHECK AND DRUG TESTING: Accepted Dental Hygiene students are
required to complete and pass the criminal background check. Instructions regarding this process will
be provided to you by the Dental Hygiene Director prior to the beginning Fall Term, first year. (Please
note: the criminal background check requires a fee and the applicant's social security number.)
    o Reasons for disqualification: - Individuals with a criminal record may not be allowed into a
        healthcare facility as a student. Because it is not possible to meet the objectives of the program
        without having clinical experience, anyone with a positive criminal or abuse history may not be
        eligible for acceptance in the Dental Hygiene program.

Dental Hygiene Admission Guidelines – Fall 2010
Admission of all students is centralized in the Admissions, Registration, and Records Office. Admission
to the College does not guarantee admission to the Dental Hygiene Program. The Admissions,
Registration and Records Office has the final authority on what constitutes equivalency for all admission
criteria and has the sole authority to inform students of their admission status.
READ THE DIRECTIONS CAREFULLY – Please note that withholding information or giving untruthful
answers to questions on this application could be cause for non-acceptance or dismissal from the
program. If you have questions regarding admission procedures please contact the Admissions,
Registration, and Records Office at 503-491-7341. Regardless of college coursework completed or
how long ago you completed the requirements, all applicants must provide the following
documentation:
A. Application Packet Checklist
B. Dental Hygiene Admission Form: (Please see Application Packet Checklist).
C. Allied Health Division application
   All applicants must submit the Allied Health Division Application form with their Dental Hygiene
   application materials.
D. $25.00 non-refundable Restricted–Entry application processing fee. (Please see Application
   Packet Checklist).
E. Personal resume
   All applicants must submit a personal resume with their application materials. Your resume should
   include both related and unrelated work experience, along with any volunteer information.
F. Program Experience/Observation form
   You must submit, by the application deadline, verification of a minimum of 20 hours of observation
   in a dental setting. Ten of these observation hours must be with a dental hygienist. Please use the
   enclosed Program Experience/Observation Form.
G. Transcripts Requirements
   All high school and college transcripts documenting required coursework listed above must be official.
   Transcripts are considered official if they are issued directly by the school or college, are mailed or
   delivered in an unopened envelope and are recently dated. If Mt. Hood Community College (MHCC)
   holds your official transcript from another college for a previous recent admission requirement, you
   may ask for that transcript to be placed in your packet. However, in some cases, it may be necessary
   to provide an updated transcript or verification of non-attendance from that college. MHCC transcripts
   will be ordered and placed in your application packet if it has been indicated as a college attended.

   Transfer coursework from foreign institutions - must be evaluated by the application deadline by an
   outside credential evaluation service. The evaluation company must be either AACRAO Foreign
   Education Credential Service (www.aacrao.org) or a member of the National Association of Credential
   Evaluation Services (NACES). For a list of NACES memberships, please refer to their web site at
   www.naces.org. The evaluation must be a course-by-course evaluation with an official copy
   submitted to the Admissions, Registration and Records Office. The coursework must be considered
   to be at a post-secondary level, have comparable MHCC accreditation status and be parallel in
   content to MHCC coursework. The outside evaluation must also be completed for secondary level
   education for any evaluation of that coursework toward MHCC admission requirements.

   Note: Admissions to the Dental Hygiene program may be rescinded if your entire educational
   history is not disclosed on this application. Additionally, MHCC will only accept transfer
   transcripts of students in “good standing.” “Good Standing” is defined as achieving at least a
   2.50 GPA in the courses relevant to being admitted into a particular program.

H. High School/Secondary Education Completion
   An official (unopened) high school transcript showing date of graduation or G.E.D. scores verifying
   completion is required, regardless of where or when this was completed and even if minimum
   high school GPA or G.E.D. composite average score is not met.
       A minimum cumulative high school GPA of 2.70 is required or a G.E.D. composite average
       score of 580 is required. If you do not meet this requirement, you may complete 14 hours or
       more of college credits earning a “C” grade or higher in the following courses*:
           Two (2) quarters of lab science (BI101 or above)
           One (1) quarter of mathematics (MTH60 or above)
           One (1) quarter of written communication skills (WR115 or above)
           *Please see the MHCC Catalog for course descriptions
     Home-schooled applicants. Home-schooled applicants must meet this requirement by completion
     of the G.E.D. test. If you are unable to submit an official high school transcript, you must complete
     the G.E.D. test. If you completed the G.E.D. and your test score results cannot be located, you
     must retake the G.E.D. test.
     High school transcripts from foreign institutions. (Please see, Transcript Requirements, for
     definition and instructions.)
     High school seniors. High school seniors are not eligible for the Dental Hygiene program unless
     they have completed college coursework in chemistry. (See, L.2) These students must submit a
     seventh semester transcript by the application deadline. If accepted into the program, verification of
     graduation must be submitted to the Admission, Registration and Records Office prior to fall term
     enrollment.
I.   College coursework
     All college transcripts must be official and recently dated. (Please see, Transcript Requirements,
     for definition of recently dated.)
     All college transcripts must indicate a minimum cumulative GPA of 2.50 or higher.
          To document the cumulative GPA, applicants must submit one current, official (unopened)
          transcript of all completed college coursework from every college or university ever attended*.
          Mt. Hood Community College transcripts will be ordered and placed in your application packet if
          it has been indicated as a college attended.
          Fall 2009 grades must be included on transcripts submitted for the Dental Hygiene program.
          Courses taken pass/fail, satisfactory/unsatisfactory or audit do not fulfill admission criteria for
          the Dental Hygiene program.
          Admissions may be rescinded if your entire educational history is not disclosed on this
          application Failure to supply all college transcripts may be considered to be fraudulent
          representation of your academic record and your application may be considered invalid.
     *NOTE: Non-accredited college transcripts will not be considered in the initial evaluation of your
     application nor can they be considered toward any degree requirements. They may however, be
     used toward proof of related education and work experience if you are invited for an interview and
     should be submitted for review.
     Post-secondary transfer coursework from foreign institutions. (Please see Transcript
     Requirements, for definition and instructions.)
J. College Grade Point Average (GPA) Requirements and Petition Process
   Your application to the Dental Hygiene program requires that you have maintained a minimum
   cumulative grade point average (GPA) of 2.50 in all completed college coursework. If you do not
   meet this GPA requirement because of coursework that you completed seven or more years ago
   you may petition to have your GPA adjusted for purposes of this admission process only.
   In order to petition for a GPA adjustment, the requirements include:

    • Your cumulative college GPA is below 2.50 for all college coursework.
    • Your college coursework was completed seven or more years ago.
    • The terms/transcripts requested in the petition must not include pre-requisites to the program.
    • You may petition for an entire term or entire transcript, but not individual classes.
    The Petition for GPA Adjustment form will need to be submitted, along with your application packet
    by the application deadline. Upon review of your petition, you will be notified in writing whether or
    not your petition has been approved.
K. Skills competency
   To be successful in the Dental Hygiene program, students need to be academically proficient.
   Students are required to demonstrate basic proficiency in the following three areas: reading,
   writing, and math. Successful completion of a writing class is not considered proof of reading
   competency for purposes of Dental Hygiene admissions. Only a reading course or CPT score will
   be used to demonstrate competency in this skill.

   You can demonstrate your skills competency through one of the following processes:
   The Mt. Hood Community College Placement Test (CPT)
   • The CPT is a non-timed skills assessment test given on a walk-in basis. There is no fee to take the
     CPT. Your scores will be available upon completion of the test.
   • CPT scores are valid for two years. If the CPT date expires on the application deadline date or
     before, the CPT will need to be taken again.
   • For purposes of admission to the Dental Hygiene program, the CPT must be taken at Mt. Hood
     Community College. Applicants who live more than two hours away may take the test by proctor.
     Please call the MHCC Testing Services for more information, 503-491-7678.
   • If you are only taking portions of the CPT you’ll need to obtain a “waiver” form by bringing
     appropriate transcripts to the Academic Advising and Transfer Center.
   • Students who choose to retest in reading, writing or math will need to complete a Retest Self-
     Referral Form, which is available in the Testing Center or Academic Advising and Transfer Center
     and pay a $10 per subject re-test fee. If your CPT scores indicate you are not ready for the Dental
     Hygiene program, you will be advised to take a course or courses in preparation to apply to the
     program at a later date. CPT scores that are older than two years at the time of Dental Hygiene
     program application are not valid and applicants will need to test for placement.
   or
   Prior College Coursework
     The CPT may be waived if an applicant has completed RD115, WR115 and MTH65 within the last
     three years (during or after 2007) with a “B” grade or higher in each (as described above).
     If an applicant has earned a bachelor’s degree or higher from an accredited school, the reading,
     writing, and math portion of the test will be waived and will be considered proficient for admission
     purposes for this program. Degree must be posted on your transcript (please see, “Transcript
     Requirements,” for definition and instructions).
     If an applicant has not earned a bachelor’s degree, prior college coursework in English and/or
     Writing does not qualify for reading competency.

L. Prerequisite/Required Preparatory Courses
   1. Chemistry requirement (15 credits with lab included)* Provide documentation of the
      following:
          Complete CH104 with a grade of “C” or higher and within seven years prior to the
          application deadline, January 22, 2010.
          Complete CH105, and CH106 (or the equivalent general chemistry series) with no grade
          lower than a “C” within seven years prior to the application deadline and must be completed
          by the end of Spring term 2010.
          o   For the chemistry cumulative gpa calculation, (see, Point Award System), all chemistry
              courses ever completed will be included in the chemistry cumulative gpa calculation.
          o   If you are currently completing the Chemistry sequence, it is your responsibility to
              provide verification of enrollment the end of each term. An official transcript verifying
              completion of the sequence with grades of “C” or higher must be submitted to the Dental
              Hygiene Admissions Evaluator by the end of Spring term 2010.
          o   Should you be accepted, failure to complete the entire chemistry series by the end of
              Spring term 2010 will result in revoking your acceptance into the program.
          o    CH103, Chemistry for Allied Health, or equivalent one-term chemistry course does not
               fulfill the chemistry requirement.
   2. Anatomy and Physiology (BI121, BI122), Microbiology (BI234), and English Composition
      (WR121) requirement.*
          Complete BI121 and BI122 with a grade of “C” or higher and within seven years by the end
          of Spring term 2010. (Note: completion date for anatomy and physiology series will change
          for 2011 admission)
          Complete BI234 with a grade of “C” or higher and within seven years by the end of Spring
          term 2010.
          Complete WR121 with a grade of “C” or higher by the end of Spring term 2010.
          o Should you be accepted, failure to complete BI122, BI234 and WR121 by the end of
               Spring term 2010 will result in revoking your acceptance into the program.
      *Please note:
          If applying for 2011 admission: BI121 and BI122 must be completed with a “C” or higher
          and within seven years prior to the 2011 application deadline.
      • In addition to chemistry, anatomy/physiology and microbiology, all other college science
          courses, for which points are assessed, must be completed with a “C” or better within seven
          years prior to the application deadline. For example, for Fall term 2010 applicants, fall term
          2003 is the oldest date any science courses can be to qualify. Exceptions may be made for
          those working in a related field.
      • Due to the rigorous demands of this program, repeated courses, especially in the sciences
          are not viewed favorably by the program faculty.
      • High school seniors are not eligible for the Dental Hygiene Program unless they have
          completed college coursework in chemistry, biology and writing as described above.
      • Applicants can meet the mathematics skills proficiency requirement for admissions by
          placing into MTH95; however, a mathematics course (minimum of MTH65), with a grade
          of “C” or better, must be completed and noted on a college transcript to meet the
          general education requirements for graduation from MHCC. (Applicants, who have
          completed a bachelor’s degree from an accredited school, refer to Skills Competency,
          above for instructions.)
      • Please see the Admissions Guidelines, Section G, for information on transcripts from
          Foreign Institutions.

M. Related Dental Hygiene experience
   It is strongly recommended that you acquaint yourself with a variety of dental hygiene settings.
   Observing, conducting informational interviews, volunteering time within the field and/or working in a
   dental office are all ways that you can gain valuable experience. The Program
   Experience/Observation Form is included in this application packet and must be faxed or mailed to the
   Admissions, Registration, and Records Office. A minimum of twenty hours of observation time is
   required for admission to the Dental Hygiene Program, ten hours of which will need to be with a dental
   hygienist. Persons with dental work experience may submit this documentation in lieu of observation.
   (Please see. This form or documented work experience must be faxed of mailed to the Admissions,
   Registration, and Records Office.)
Points Assessment
All application packets that are considered complete (submitted all required materials) by the
application deadline will be evaluated and awarded points to determine the top 50 candidates to be
interviewed. Those candidates interviewed will be awarded points for their written and verbal responses
in the interview. The interview points will be added to the pre-interview points to determine the final
candidates to be accepted into the Dental Hygiene program, those placed on the alternate list and
those encouraged to re-apply the following year. Please see the Point Award System within the
application packet to view the categories where points are awarded, amount of points for each category
and to estimate your pre-interview points.
Points are assessed for each category noted below:
Points are only awarded for courses taken through Fall 2009.
Pre-interview (please see Point Award System for further information):
A. High school cumulative grade point average (GPA) or the G.E.D. composite score plus GPA
   for chemistry, biology and mathematics. (20 points maximum). High school seniors are not
   eligible for the Dental Hygiene Program unless they have completed college coursework in
   chemistry. (See K.2)
B. College cumulative GPA plus GPA in chemistry, biology and mathematics. (28 points
   maximum). Chemistry and other college science courses must be completed with a “C” or higher
   within seven years prior to the application deadline. Exceptions may be made for those working in
   a related field. All grades from repeated courses are included in the GPA computation.
C. Residency considerations. (3 points maximum). The Dental Hygiene Program is now accepting
   applications from out-of-state.
D. Reapplication. (5 points maximum).
   Note: Applicants who were previously accepted into the program and then withdrew or declined
   acceptance are not guaranteed priority consideration. If you previously applied for a program, you
   must reapply for the program following the current application packet materials. We recommend that
   you send recently dated official transcripts. Any transcripts received in prior years cannot be
   guaranteed to be on file. Additionally, students, who have been previously dismissed from MHCC’s
   Dental Hygiene program or any other health related program, may be denied admission into the
   program.
E. Other. (10 points maximum).
F. Work experience (point value determined by program faculty at interview)
G. Group interview (point value determined by program faculty at interview)
   The top 50 applicants will be invited to attend a group interview with program staff. This is a professional interview.
   Applicants must be prepared to respond to questions verbally and in writing. Written responses will be evaluated for
   neatness, legibility, grammar, spelling, and content.

BOARD EXAMS AND/OR LICENSING
You must be licensed to practice in Oregon and any other state. In order to be licensed in Oregon
students must pass two types of licensing exams. It is required that all graduates take these exams
upon successful completion of the program.

Students who have been previously dismissed from MHCC’s Dental Hygiene program or any other
health related program may be denied admission into the program.

Applicants to Allied Health Programs must be physically and mentally able to cope with the rigors of the
curriculum and the demanding nature of the health professions. Established academic and clinical
requirements essential to the program of instruction apply to all students and cannot be waived.

2009 Application Pool Statistics: For the program beginning Fall term 2009 there were 144
applications received. The top 50 applicants were invited for an interview. The minimum number of
points needed to attend the interview was 41 points. 18 applicants were accepted into the program.

      c. Obtain, review and update a complete medical, family, social and dental history.

Diagnosis
PC.2 Use critical decision making skills to reach conclusions about the patient’s/client’s dental hygiene needs based on all
      available assessment data.
      This competency includes:
      a. Use assessment findings, etiologic factors and clinical data in determining dental hygiene diagnosis.
      b. Identify patient/client needs and significant findings that impact the delivery of dental hygiene services.
      c. Obtain consultations as indicated.
Planning
PC.3 Collaborate with the patient/client, and /or other health professionals, to formulate a comprehensive dental hygiene
      care plan that is patient/client-centered and based on current scientific evidence.
      This competency includes:
      a. Prioritize the care plan based on the health status and the actual and potential problems of the individual to facilitate
          optimal oral health.
      b. Establish a planned sequence of care (educational, clinical and evaluation) based on the dental hygiene diagnosis;
          identified oral conditions; potential problems; etiologic and risk factors; and available treatment modalities.
      c. Establish a collaborative relationship with the patient/client in the planned care to include etiology prognosis, and
          treatment alternative.
      d. Make referrals to other health care professionals.
      e. Obtain the patient’s/client’s informed consent based on a thorough case presentation.
Implementation
PC.4 Provide specialized treatment that includes preventive and therapeutic services designed to achieve and maintain oral
      health. Assist achieving oral health goals formulated in collaboration with the patient/client.
      This competency includes:
      a. Perform dental hygiene interventions eliminate and/or control local etiologic factors to prevent and control caries,
          periodontal disease and other oral conditions.
      b. Control pain and anxiety during treatment through the use of accepted clinical and behavioral techniques.
