Complete and mail entire registration form (one form per person – photocopy form for additional registrants). Complete all information
required on form. Circle the appropriate fee for each registration choice. Total the amount due and submit the remittance fee along with the
completed registration form. Registration form must be accompanied by check, credit card number, or purchase order. To avoid duplicate
charges, DO NOT mail original after faxing form. If original signed purchased order is required please mail purchase order and registration
form DO NOT fax. Checks should be made payable to NJWEA in US funds.
Cancellation must be in writing. No phone calls will be accepted. There will be a 25% charge for cancellations made prior to April 18, 2005.
Absolutely no refunds will be made after this date.
Purchase orders are accepted from US governmental agencies. Purchase orders must be approved with all authorized signatures and the
purchase order number referenced in the payment section of the registration form. Include a copy of the purchase order with the
registration form. If an original signed purchase order is required please mail purchase order and registration form, DO NOT fax.
Pre-Registration/On-Site Registration: Pre-registration credentials and on-site registration is located on the 4th Floor, North Tower,
Tropicana Resort and Hotel, Atlantic City. Pre-Registration confirmations are not mailed to registrants. If confirmation is required, please
