To All Suppliers: For the products supplied to us by your firm, please forward to my associate’s attention, as noted below, a current Certificate of Product Liability, indicating the total amount of your Product Liability Coverage. The name and address on the certificate should be stated as follows: CVS CORPORATION One CVS Drive, Vendor #«Vendor_Number» Woonsocket, Rhode Island 02895 ATTN: SHANNON ORTIZ THIS CERTIFICATE MUST SATISFY THE FOLLOWING REQUIREMENTS: • Name insured must be company with whom CVS is doing business (not parent company/broker name). • BODILY INJURY- each occurrence/aggregate. • PROPERTY DAMAGE - each occurrence/aggregate. • CVS requires all vendors to carry a minimum of $5 million per occurrence for general liability coverage. This coverage may be a combination of Primary, plus Excess/Umbrella coverage. • BLANKET VENDORS ENDORSEMENT COVERAGE. • CVS CORPORATION must be named as ADDITIONAL INSURED. • CANCELLATION POLICY SHOULD READ AS FOLLOWS; ‘Should any of the above described policies be canceled before the expiration date thereof, the issuing company will mail thirty (30) days written notice to the named certificate holder”.
