DRUG AND ALCOHOL TESTING FORMS TO BE USED (1000)
(No. 118 August 2008) The testing process for the State and Federal programs is basically the same. However, the forms that must be used are different, as explained below: Federal Testing Program Applies to classes requiring commercial driver’s license (except restricted fire fighter license). Includes reasonable suspicion, random, pre-employment, post accident, return to duty testing, and follow-up testing. The clinics and mobile collectors do not have blank copies of these forms. Form DPA-177 (Rev. 12-07) – Drug/Alcohol Test Authorization - This form must be prepared for all drug/alcohol tests and given to the clinic/mobile collector performing the test. The form includes the employee’s consent to be tested. KROLL Federal Drug Testing Custody and Control Form (pre-printed Facility Number 271939) - This form is used for all federal drug tests. KROLL U.S. Dept. of Transportation Alcohol Testing Form (pre-printed Facility Number 271939) - This form is used for all federal alcohol tests. State Testing Program Applies to classes identified as sensitive excluding the employees covered under the federal program. Only reasonable suspicion tests can be done under state testing. The clinics and mobile collectors do not have blank copies of these forms. KROLL U.S. Dept. of Transportation Alcohol Testing Form (pre-printed Facility Number 271964) - This form is used for all non-federal alcohol tests. (The federal and nonfederal alcohol testing forms look the same except for the facility number. Make sure that you check the facility number and use the correct form.)
